腔内超声对卵巢储备功能下降患者促排卵治疗前后卵巢功能及妊娠率的观察

Ovarian function and pregnancy rate in patients with diminished ovarian reserve before and after ovulation induction by transvaginal ultrasoography

目的：采用经阴道彩色多普勒超声检查观察卵巢储备功能下降（DOR）患者经促排卵治疗前后,卵巢功能及妊娠率的变化。方法：40例DOR患者于自然月经周期第2-3天测定基础性激素水平,同时经阴道彩色多普勒超声检测卵巢最大切面平均直径（MOD）、窦卵泡数（AFC）、卵巢间质动脉峰值流速（PSV）、阻力指数（RI）,之后行促排卵治疗,超声监测卵泡发育,应用人绒毛膜促性腺激素（hCG）诱导排卵,记录HCG日优势卵泡数目及上述测量数值,指导同房或宫腔内人工授精（IUI）。结果 ：促排卵治疗后卵巢MOD、AFC、PSV、RI和h CG日所获优势卵泡数及妊娠率均显著高于治疗前（P〈0.05）。结论：促排卵治疗有助于改善卵巢储备功能,获得优势卵泡,提高妊娠率。

Objective To investigate the ovarian function and pregnancy rate of patients with diminished ovarian reserve （DOR） before and after ovulation induction by transvaginal color doppler ultrasoography. Methods Fourty patients with DOR, during the second and third day of their natural menstrual period, received detections of the basic sexual hormone level, the maximum ovarian diameter （MOD）, antral follicle count （AFC）, peak systolic velocity （PSV） and resistance index （RI）. Then ovulation induction treatment was performed with mornitoring of the developing follicles. The dominant follicles and the related parameters were recorded during the human ehorionic gonadotrophin （hCG） adminstration. After the administration of hCG, the patients were given guided intercourse or intrauterine insemination （IUI）. Results After ovulation induction treatment, ovarian MOD, AFC, PSV, RI, the number of dominant follicles in the hCG day and the pregnancy rate were significantly increased compared with those before ovulation induction （P 〈 0.05,respectively）. Conclusion Ovulation induction treatment contributes to gaining more dominant follicles and improving pregnancy rate in DOR patients.
