磁敏感加权成像对急性脑梗死溶栓治疗后再灌注损伤的评价价值 被引量:4

摘要 目的比较磁敏感加权成像(susceptibility weighted imaging,SWI)与常规MRI序列对急性脑梗死溶栓治疗后再灌注损伤的病灶显示情况,探讨SWI对急性脑梗死再灌注损伤的临床应用价值。方法临床确诊为急性脑梗死的患者40例,治疗前先行常规MRI及弥散加权成像(diffusion weighted imaging,DWI),溶栓治疗后再行CT、常规MRI及SWI检查。分别观察脑梗死溶栓治疗后病灶内有无出血,出血程度及梗死区周边小血管的变化,再采用U检验及配对t检验进行分析。结果 40例急性脑梗死患者溶栓治疗后随访中,SWI发现梗死后出血29例(72.5%)。常规MRI检查发现梗死后出血24例(60%),两种检查方法在评价急性脑梗死溶栓治疗后再灌注损伤敏感性方面具有统计学意义(Z=-6.721,P=0.003)。常规MRI序列和SWI显示出合并出血的24例患者中,出血量的总体积分别为(42.57±0.023)cm^3、(50.25±0.037)cm3,平均体积分别为(1.77±0.014)cm3、(2.09±0.017)cm3。两种方法在检测病灶总体积方面差异具有统计学意义(t=3.03,P=0.031)。结论 SWI在显示急性脑梗死溶栓治疗后再灌注损伤的敏感性及评价患者病情方面均优于常规MRI序列。
