多种超声检查模式对晚期宫颈癌治疗前评估的价值分析

Value Evaluation Prior Treatment of Various Ultrasound Examination Methods Locally in Advanced Cervix Cancer

目的探讨多种超声检查模式对晚期宫颈癌治疗前评估的价值。方法对21例Ⅱb及以上分期的晚期宫颈癌患者进行治疗前的多种模式(常规超声、弹性超声、超声造影)超声检查,在不同检查模式下测量病变宫颈的大小(长径、前后径)并加以比较。超声造影分析宫颈的时间-强度曲线(TIC)。结果晚期宫颈癌宫颈体积均明显增大,显著大于对照组。宫颈癌组二维超声、弹性超声、超声造影三种模式的宫颈测量值比较无明显差异(P>0.05);晚期宫颈癌病变区弹性评分≥4分,对照组宫颈弹性评分≤3分,晚期宫颈癌组有4例因局部液化弹性超声见BGR现象;晚期宫颈癌超声造影使病变区边界更加清晰易于识别,可通过无灌注区识别液化坏死区。结论多种超声检查模式的联合应用对晚期宫颈癌原发病变情况的评估全面可靠;超声造影模式更具评估价值。

Objective To approach the clinical value of various ultrasound examination methods in evaluating locally advanced cervix cancer before treatment. Methods All the participants who staged lib or higher take conventional ultrasound, ultrasonic elastography, con- trast - enhanced ultrasonography before operation. Results The cervical volume of cervical cancer was larger than control group. （ long di- ameter 5.10 ± 0. 11 cm vs 2. 89 ±0. 32 cm, antero - posterior diameter 4. 82 ± 0. 14 em vs 2.43±0. 28 era） （ P 〈0. 05 ）. The cervical volumes in 2 - D color doppler ultrasound, ultrasonic elastography and contrast - enhanced ultrasonography were not statistically different（ P 〉 0. 05）. Elasticity score of locally advanced cervix cancer was greater than or equal to 4, and that of benign lesions was less than or equal to 3. ＂Blue green red （BGR） ＂ was obsevered in four of case groups in consequence of liquefactive necrosis. Contrast - enhanced uhrasonog- raphy made boundary 〈 javascript：void （0） ; 〉 of locally advanced cervix cancer distinct. Non - peffusion area was obversed in liquefactive necrosis. Conclusion The combined application of various ultrasound examination methods could provide more information for locally ad- vanced cervix cancer before treatment. Contrast- enhanced ultrasonograohy contributes more.
