心理护理配合人文关怀对乙肝患者负性心理的影响研究

The effect of psychological nursing with humanistic care on hepatitis B patients with negative mental status

目的 观察并探讨心理护理配合人文关怀的护理模式对慢乙肝（ Chronic Hepatitis B,CHB）患者负性心理和治疗依从性的改善作用. 方法 我院自2013年5月至2014年4月起对接诊的100例CHB患者在常规护理的基础上给予心理护理配合人文关怀的护理模式,作为干预组;选取此前一年在我院接受治疗和常规护理的100例CHB患者作为对照组. 护理前后利用汉密尔顿抑郁评分量表 （ HAMD）、汉密尔顿焦虑评分量表（ HA-MA）和匹兹堡睡眠质量指数量表（ PSQI）对两组患者的抑郁、焦虑和睡眠质量进行评估,并统计两组患者的治疗依从性. 结果 两组患者护理前HAMD、HAMA和PSQI得分相比,差异无统计学意义（ P〉0. 05 ）;护理后,干预组HAMD、HAMA和PSQI分别为（22. 8±6. 8）分、（25. 2±8. 4）分和（24. 3±9. 3）分,对照组为（43. 1±9. 3）分、（40. 7± 10. 8）分和（38. 6±10. 2）分,两组相比,差异具有统计学意义（P〈0. 05）;护理后,干预组戒烟戒酒、遵医用药、生活规律和定期复查的比例均明显高于对照组,即治疗依从性明显高于对照组（ P〈0. 05 ）. 结论 心理护理配合人文关怀能明显改善CHB患者的心理状况和睡眠质量,有利于患者治疗依从性的提高.

Objective To observe and explore the effect of psychological nursing combine with humanistic care on hepatitis B （ CHB） patients with negative mental status. Methods A total of 100 cases of patients with CHB were given psychological nursing with humanistic care nursing intervention on the basis of the routine care in our hospital from May 2013 to April 2014 as the intervention group, while 100 cases with CHB in our hospital for treatment and nursing at previous year were selected as control group. Hamilton Depression Rating Scale （ HAMD） , Hamilton Anxiety Rating Scale （ HAMA） and Pittsburgh Sleep Quality Index Scale （ PSQI） were used to assess the depression, anxiety and sleep quality of the two groups. The treatment compliance of the two groups were recorded and compared. Results The scores of HAMD, HAMA and PSQI in the two groups before nursing were not statistically significant （ P〉0. 05） . The scores of HAMD, HAMA and PSQI in the intervention group were （22. 8 ± 6. 8） points, （25. 2 ± 8. 4） points and （24. 3 ± 9. 3） points, while in the control group were （43. 1 ± 9. 3） points, （40. 7 ± 10. 8） points and （38. 6 ± 10. 2） points, the differences were statistically significant （P〈0. 05） . The ratio of quit smoking and drinking, compli-ance of medication, the law of life, and the proportion of regular review in intervention group after care were significantly higher than those of control group （ P〈0. 05 ） . Conclusion Psychological nursing combine with humanistic care inter-vention can significantly improve the psychological status and sleep quality of CHB patients, beneficial to the patients treatment compliance.
