摘要 目的:探讨应用组织工程技术中的支架技术和生长因子技术,在上颌后牙区重建种植体周围骨组织与软组织的组织学表征。方法:选择上颌后牙区牙缺失伴骨量不足患者20例,随机分实验组对照组;实验组非细胞型组织工程技术重建上颌种植体周围组织。对照组使用常规重建种植体周围组织。术后6月二期术取样作组织学、能谱分析和免疫组织化学等检查。结果:实验组边缘骨组织形态学特征、骨缺损指数、成骨细胞指数、钙磷元素质量百分比及钙磷比值、骨形成蛋白(BMP)平均灰度优于对照组(P<0.05)。结论:非细胞型组织工程技术可有效重建上颌种植体周围组织,对种植修复的长期成功率具有重要的意义。 Objective: To evaluate the effects of cell free-tissue engineering technique in maxillary peri-implant tissue reconstruction. Methods: A randomized controlled experiment was conducted. 20 adult patients with two or three teeth loss and bone defect in posterior maxilla were divided into two groups, experimented groups and controlled groups. Experimented groups were adopted cell free-tissue engineering technique to reconstruct maxillary peri-implant tissue, while controlled groups were adopted the regular one. Finally, implant-supported prosthetics were applied in both groups. Effect were analyzed by histological and immunohistochemical technique, and energy spectrum analysis. Results: Histological character of marginal bone, bone defect index, osteoblast index, mass fraction proportion and rate of calcium and phosphorus and average grey degree of bone morphogenetic protein (BMP) in experimented groups are better than the controlled one(P&lt;0.05). Conclusion: Cell free-tissue engineering technique could reconstruct maxillary peri-implant tissue effectively and would contribute a lot in improving the long-term success rate of dental implant prosthetics.
