《外科正宗》消风散临床应用经验浅探

Brief Discussion on Clinical Application of Xiaofeng Powder of “Waike Zhengzong”

[目的]探讨《外科正宗》消风散在治疗皮肤病中的临床应用。[方法]搜集笔者近几年应用消风散治疗皮肤病的医案,选择疗效比较显著的病例,从应用指征、病机、用药特点几个层面进行分析,总结用药特点和规律。[结果]消风散应用指征主要有皮肤瘙痒、皮疹颜色鲜红、小便色黄、大便干结、舌质红、苔黄等,病机为外风侵袭肌肤、内有血虚和血热,用药特点为疏风止痒与滋阴凉血并用。验案中风热疮患者,辨证为风热血燥,治以疏风清热、凉血止痒;风瘙痒患者,辨证为血虚化燥生风,治以养血润燥、祛风止痒;丘疹性荨麻疹患者,辨证为血热生风,治以凉血祛风止痒,以消风散加减治疗,均获良效。[结论]消风散集疏风、清热、祛湿、养血、滋阴多种治法于一体,临床选用此方加减治疗常见皮肤病疗效显著。

[Objective]To discuss the clinical application of Xiaofeng powder of'Waike Zhengzong'in treating skin diseases.[Methods]Through selecting a few clinical cases with remarkable effect in recent years using Xiaofeng powder,the characteristics and regularity of medication was summarized from aspects such as indications of application,pathogenesis and medication characteristics.[Results]Indications of application of Xiaofeng powder consist of skin pruritus,scarlet rash,yellow urine,dry and hard excrement,red tongue,yellow tongue coating and so on.Pathogenesis is external wind invading skin,as well as blood deficiency and blood heat inside the body.The medication characteristic is combination of dispelling wind,relieving itching and nourishing Yin,cooling blood.The patient with wind-heat sores was differentiated as wind heat and blood dryness,followed the principle of dispelling wind,clearing heat,cooling blood and relieving itching.The patient with pruritus cutanea was differentiated as deficiency of blood,transformation of dryness into wind,followed the principle of nourishing blood,moistening dryness,dispelling wind and relieving itching.The patient with urticaria was differentiated as blood heat generating wind,followed the principle of cooling blood,dispelling wind and relieving itching.All patients achieved good results after treated with modified Xiaofeng powder.[Conclusion]Xiaofeng powder has multiple effects in dispelling wind,clearing heat,eliminating dampness,nourishing blood,nourishing Yin and so on,and the clinical curative effect is remarkable.
