蔡宛如名中医治疗慢阻肺气道粘液高分泌临床经验

CAI Wanru’s Clinical Experience in Treating Chronic Obstructive Pulmonary Disease(COPD) with Airway Mucus Hypersecretion

[目的]总结浙江省国医名师蔡宛如教授治疗慢阻肺气道(下同)粘液高分泌的学术思想及临证经验。[方法]通过临床随诊抄方,并查阅相关文献,列举典型病案,总结蔡师治疗慢阻肺气道黏液高分泌的辨证思维及遣方用药规律。[结果]蔡宛如教授认为痰瘀阻肺是慢阻肺气道黏液高分泌的致病核心,并根据患者的不同分期,急性加重期重在祛痰,主张祛风止痉、清热化痰、活血行气;缓解期治在补虚,主张益气健脾、滋阴润肺、温补肾阳,对抑制慢阻肺患者气道黏液高分泌具有很好的疗效。[结论]蔡宛如教授长期从事呼吸系统疾病的预治工作,临床经验丰富,其治疗慢阻肺气道黏液高分泌的经验值得学习研究。

[Objective]To summarize the academic thoughts and clinical experience of Professor CAI Wanru,a famous doctor in Zhejiang Province,in treating airway mucus hypersecretion of COPD.[Methods]Through the clinical follow-up transcription,and consult the relevant literature,enumerate the typical medical records,sum up the dialectical thinking of Teacher CAI in the treatment of high-secretion mucus of chronic obstructive pulmonary airway and the law of medication.[Results]Professor CAI Wanru believes that obstruction of the lungs is the core of the high secretion of mucous membranes in the chronic obstructive pulmonary airway,and according to the different stages of the patients,the acute exacerbation period is heavy in the phlegm,and it is advocated that the phlegm and blood stasis,clearing away heat and phlegm,promoting blood circulation and Qi;Relieving period in the treatment of deficiency,advocates Qi and spleen,nourishing Yin and moistening the lungs,warming the kidney and Yang,has a good effect on inhibiting airway mucus hypersecretion in patients with COPD.[Conclusion]Professor CAI Wanru has been engaged in the pre-treatment of respiratory diseases for a long time and has rich clinical experience.Her experience in treating airway mucus hypersecretion of COPD deserves our study.
