高频彩色多普勒超声诊断膝关节滑膜炎的临床价值

Clinic value of high frequency color dopper ultrasonic diagnosis in gonarthromenigitis

目的 探讨高频彩色多普勒超声诊断膝关节滑膜炎的声像图特点及其在临床的应用价值。方法 回顾性分析我院和南方医科大学第三附属医院自2014年1月-2015年9月收治的45例诊断为膝关节滑膜炎患者,分析高频彩色多普勒超声声像图特点,并与X线平片检查作比较。结果 高频超声在滑膜厚度、关节腔积液、软骨病变等方面具有非常良好的分辨率,检出率分别为95.56%、93.33%、31.11%;结合彩色多普勒,在滑膜炎活动期,其血流分级为Ⅱ-Ⅲ级,其与滑膜动脉阻力指数（RI）呈高度负相关。结论 高频彩色多普勒超声对于滑膜炎的诊断具有良好的临床价值,值得推广与应用。

Objective To analyze the clinical application value of gonarthromenigitis by high frequency color doppler ultrasonic diagnosis and to conclude the characters of gonarthromenigitis ultrasonography.Methods A retrospective analysis was made on the clinical data of 45 cases of gonarthromenigitis in our hospital and the Third Affiliated of Southerm Medical University from Janury 2014 to September 2015.The characteristics of high frequency color doppler ultrasound images were analyzed.A comparison was made between the results from high frequency color doppler and X-ray plain film onspection.Results In detecting the changes of synovial hypertrophy,arthroedema,knee cartilage affection,the ultrasonography had satisfactory resolution value.The detection rates of above changes by ultrosonography were 95.56%,93.33%,31.11%respectively.During the gonarthromenitigi active stage,the flow classification was Ⅱto Ⅲ according to the color doppler.An excellent colleration with synovial（RI）was showed.Conclusion High frequency color doppler ultrasonography is useful in the diagnosis of gonarthromenigitis and is worth widing clinical application.
