摘要
目的探讨分析关节镜下不可吸收缝线固定治疗前交叉韧带胫骨止点撕脱骨折临床效果。方法将2014年1月至2015年10月30例前交叉韧带胫骨止点撕脱骨折患者作为实验组,应用关节镜下不可吸收缝线固定进行治疗,同时选取2012年1月至2014年12月30例同样骨折类型的患者作为对照组,应用经髌旁的长弧形切口进行复位固定。观察并比较两组患者经过治疗后的疗效、手术前后膝关节功能评分变化情况、手术时间、手术后功能活动时间和住院时间。结果根据膝关节Lysholm评分系统评估,实验组患者治疗后3、6、12、18个月Lysholm评分均较治疗前升高,差异有统计学意义(P<0.05)。实验组患者的手术时间、术后功能活动时间和住院时间均明显少于对照组患者,差异有统计学意义(P<0.05)。实验组患者并发症发生率明显低于对照组,差异统计学上有意义(P<0.05)。结论关节镜下不可吸收缝线固定治疗前交叉韧带胫骨止点撕脱骨折,不仅微创美观,而且固定牢靠,有利于早期功能锻炼,减轻了患者的经济负担和手术痛苦,具有广阔的临床应用前景。
Objective To investigate the clinical effect of non absorbable suture fixation in the treatment of anterior cruciate ligament tibial avulsion fracture in arthroscopic. Methods From January 2014 to October 2015,30 cases of anterior cruciate ligament avulsion fracture of the tibia were taken as the experimental group. The patients in the experimental group received non absorbable suture arthroscopic fixation for treatment. At the same time,30 cases of the same type of fracture patients were included as the control group,from January 2012 to December 2014.The application of the para- patellar long curved incision included reduction and fixation. The efficacy of the knee joint function score changes,operation time,postoperative functional activity time and hospital stay after treatment of two groups are compared. Results According to Lysholm knee score system evaluation,Lysholm score of patients in the experimental group before treatment were higher than 3,6,12,18 months after treatment,with statistically significant difference( P〈0. 05). The operation time,postoperative activity time and hospitalization time in the experimental group with were significantly less than the control group patients,the difference was statistical significant( P〈0. 05). The incidence of complications of patients in the experimental group was significantly lower than the control group,the difference was statistically significant( P〈0. 05). Conclusion Non absorbable suture fixation under arthroscopy in the treatment of anterior cruciate ligament avulsion fracture,not only minimally invasive,but also stable and reliable,is conducive to early functional exercise,reduce the economic burden of patients and surgical pain,has broad clinical application prospects.
