复杂胫骨平台骨折手术预后影响因素分析

The analysis of influencing factors of operation effect in patients with complicated tibial plateau fractures

目的分析复杂胫骨平台骨折手术疗效的影响因素。方法回顾分析2010年1月至2014年1月期间收治的76例复杂胫骨平台骨折患者的临床资料,对影响手术疗效的因素进行分析。结果 76例胫骨平台骨折患者均康复出院,复位满意患者的术后疗效优良率为89.89%,显著高于不良复位的患者,差异具有统计学意义(P<0.05)。76例复杂胫骨平台骨折患者术后共有19例患有切口感染及坏死等明显并发症,并发症发生率为25.00%。对影响复杂胫骨平台骨折的因素进行统计学分析表明,致伤原因、手术时机、术后并发症、骨折类型、复位质量是手术疗效的独立影响因素(P<0.05)。结论影响复杂胫骨平台骨折的主要因素有骨折类型、复位质量、手术时机、致伤原因等,提高关节面复位质量及内固定牢靠程度有助于获得更加满意的手术效果。

Objective To analyze the influencing factors of operation effect in patients with complicated tibial plateau fractures. Methods There were 76 cases of patients with complex fractures of tibial plateau in our hospital between January 2010 and January 2014. The clinical data of patients were retrospectively analyzed and analyzed the influencing factors about the curative effect of surgery. Results The 76 cases of patients recovered from the fracture in this study. The curative effect rate in reset satisfaction patients was 89. 89%,which was significantly more than bad reset patients（ P〈0. 05）. There were 19 cases of patients with obvious complications in 76 cases of patients with complex tibial plateau frac-tures. The incidence of complications was 25. 00%. The influential factors for curative effect of complex fractures of tibial plateau was analyzed,which showed the causes,operation time,fracture type and complications had significant influence and had statistically significant difference（ P〈0. 05）. Conclusion Discouraged results of complicated tibial plateau fractures are significantly related to the causes,operation time,fracture type and complications. Improving the quality of articular surface reduction and internal fixation is helpful to get more satisfied effect of surgery.
