益气温阳活血方对慢性心力衰竭心室重塑患者外周血单个核细胞Bax/Bcl-2表达的影响

Effects of Yiqiwenyanghuoxue decoction on Bax/Bcl-2 of PBMC in chronic heart failure patiens with cardiac remodeling

目的探讨益气温阳活血方对慢性心力衰竭心室重塑患者外周血单个核细胞(PBMC)BCL-2相关X蛋白(BCL2-Associated X Protein,Bax)/B细胞淋巴瘤/白血病-2(B cell lymphoma/lewkmia-2,Bcl-2)表达的影响。方法将慢性心力衰竭患者122例随机分为治疗组和对照组,两组均按慢性心力衰竭治疗指南给予常规治疗,治疗组加服用益气温阳活血方,治疗共3个月。两组治疗前后分别以心脏超声检查患者左心室收缩末径(LVESD)、舒张末径(LVEDD)和左心室射血分数(LVEF),逆转录聚合酶链反应(RT-PCR)法以及蛋白免疫印记法(Western-blot)检测Bax、Bcl-2 m RNA及蛋白质表达水平。结果 3个月后,与治疗前比较,对照组、治疗组治疗后LVESD、LVEDD、Bax m RNA及蛋白质表达水平降低,而LVEF、Bcl-2 m RNA及蛋白质表达水平则升高,差异均有统计学意义(P<0.05),且治疗组治疗后上述指标与对照组治疗后比较进一步改善,差异均有统计学意义(P<0.05)。结论益气温阳活血方通过下调Bax基因表达以及上调Bcl-2基因表达抑制慢性心力衰竭患者心室重塑,改善心脏功能。

[Objective]To study the effects Yiqiwenyanghuoxue decoction on Bax/Bcl-2 of PBMC in the cardiac remodeling of patients with chronic heart failure. [Methods]122 patients with chronic heart failure were randomly divided into control group and treated group. The control group were given conventional treatment, the treated group were given Yiqiwenyanghuoxue decoction and combined routine therapy, three months was taken as a course of treatment. LVESD, LVEDD and LVEF were measured by ultrasonic cradiography, m RNA and protein levels of Bax and Bcl-2 were evaluated with RT-PCR and Western-blot respectively. [Results] After 3 months, LVEDD,LVESD, m RNA and protein levels of Bax were decreased while LVEF, m RNA and protein levels of Bcl-2 were increased in both therapy group compared with those pre-therapy（P〈0.05, respectively）, and the above indicators got further improvement in treated group compared with control group（P〈0.05, respectively）. [Conclusion] Yiqiwenyanghuoxue decoction could inhibit the myocardial remodeling through down-regulating Bax and upregulating Bcl-2.
