射频热凝术治疗三叉神经痛的临床效果观察

Clinical Effect of Radiofrequency Thermal Coagulation in the Treatment of Trigeminal Neuralgia

目的:观察并分析射频热凝术治疗三叉神经痛的临床疗效。方法:选取2009年3月-2014年2月于本院就诊的90例三叉神经痛患者,按照随机数字表法将其分为观察组(射频热凝术治疗)与对照组(伽玛刀法治疗),每组各45例。观察比较两组患者的治疗效果、并发症和复发情况以及术后1周、1个月、3个月的VAS评分。结果:经治疗后,观察组患者的治疗总有效率93.33%明显高于对照组的77.78%,差异有统计学意义(字2=4.4056,P=0.0358)。观察组术后的并发症发生率与1年后复发率分别为13.33%、2.22%,均明显低于对照组的31.11%、17.78%,差异均有统计学意义(字2=4.1143,P=0.0425;字2=4.4444,P=0.0350)。观察组术后1周、1个月、3个月的VAS评分均明显低于对照组(t=10.06、9.76、10.87,P<0.05)。结论:应用射频热凝术治疗三叉神经痛的效果优良,可在较短时间内缓解疼痛,且疗效显著,安全性高,能有效降低并发症发生率,具备临床推广应用的价值。

Objective：To observe and analyze the clinical effect of radiofrequency thermal coagulation in the treatment of trigeminal neuralgia.Method：90 patients with trigeminal neuralgia admitted to our hospital from March 2009 to February 2014 were selected and divided into the observation group（radiofrequency thermal coagulation therapy） and the control group（gamma knife therapy） according to the random number table method,45 cases in each group.The treatment effect,complications,recurrence and VAS scores postoperative 1 week,a month,three months between the two groups were observed and compared.Result：After treatment,the total effective rate of the observation group was 93.33%,which was significantly higher than 77.78% of the control group,the difference was statistically significant（ χ^2=4.4056,P=0.0358）.The postoperative complication rate and recurrence rate after 1 year of the observation group were 13.33%,2.22%,which were significantly lower than 31.11% and 17.78% of the control group,the differences were statistically significant（ χ^2=4.1143,P=0.0425; χ^2=4.4444,P=0.0350）.The VAS scores of the observation group postoperative 1 week,a month,three months were significantly lower than those of the control group,the differences were statistically significant（t=10.06,P〈0.05;t=9.76,P〈0.05;t=10.87,P〈0.05）.Conclusion：The application effect of radiofrequency thermal coagulation in the treatment of trigeminal neuralgia is excellent,it can relieve pain in a relatively short time,and has a significant curative effect,with high safety,can effectively reduce the rate of complications,with the value of clinical application.
