喉癌全喉切除术后食管发音参数检测的循证研究

Evidence-based research on detection of esophageal speech parameter in patients after total laryngectomy

[目的]系统评价喉癌全喉切除后食管发音参数检测的方法。[方法]计算机检索万方、维普、中国知网(CNKI),纳入相关研究文献,对纳入研究基本特征及质量评价、参数检测的仪器、参数、效果评价几方面进行分析。[结果]共纳入文献7篇,食管发音训练后声音强度、发音频率、呼气流率、最长发声时间、基频、基频微扰、振幅、振幅微扰、谐噪比与对照组比较差异有统计学意义(P<0.05),且与正常发音多项参数比较差异有统计学意义(P<0.05)。[结论]食管发音参数检测的仪器、参数多样,有待规范;通过参数检测能客观评价发音训练效果,指导食管发音训练,为进一步进行嗓音康复工作提供依据。

Objective：To systematically evaluate the pronunciation parameter detection method laryngeal cancer after total laryngectomy esophageal.Methods：Computer retrieval was used in Wanfang,VIP,CNKI,included the related research nature of literatures.Then the basic features included in the study and quality evaluation,parameters inspection instruments,parameters and effect evaluation were analyzed.Results：Totally 7literatures were included.There was statistically significant difference in the intensity of the sound,voice frequency,expiratory flow rate,the longest time,fundamental frequency and fundamental frequency perturbation,amplitude,amplitude perturbation and harmonic to noise ratio after esophageal pronunciation training（P〈0.05）,and they were different with several parameters of normal pronunciation（P〈0.05）.Conclusion：The esophagus pronunciation parameter testing instrument,and the diversity of parameters need to be regulated;parameter detection could objectively evaluate the efficacy of the pronunciation training,guide the esophageal pronunciation training,and provide the basis for further voice rehabilitation work.
