多普勒血流谱结合二维图像测量左室射血分数

摘要 目的:探讨利用Doppler技术测量每搏量(SV)结合二维(2D)超声技术测量左室舒张末容积(LVEDV)以测定左室射血分数(LVEF)的价值。方法:分别用M超、2D单平面法和Doppler与2D实时超声技术结合法(TVI法)测量了37例左室形态和功能明显变化心脏病人的LVEF,并与24h内完成的放射性核素心血池造影(NCT)测定的LVEF进行比较。结果:TVI法与2D、M法测量的LVEF均值差异显著(0.37±0.2:0.46±0.12,P<0.05,0.37±0.2:0.47±0.08,P<0.05),而与NCT法相近(0.37±0.2:0.37±0.17,P>0.05),3种UCG方法所测LVEF均与NCT所测的LVEF值相关,M和2D测定高估LVEF,而TVI法值近于NCT值,相关性以TVI值最好,2D次之,M法最差(r值分别为0.957、0.834和0.752)。结论:在左室形态和功能明显变化的情况下,TVI方法测量LVEF明显优于目前临床常用的M和2D单面面积一长轴法。 Objective: To study the left ventricular ejection fraction (LVEF) could be estimated more accurately by stroke volume (SV) of Doppler technique combined left ventricular end diastolic volume (LVEDV) of two-dimension (2D) method in echocardiogram for patients with remarkable functional and morphologic changes of left ventricle. Methods: LVEF were measured by M、2D method and Doppler technique combined with 2D (TVI) in 37 patients with left ventricle enlargement. who underwent cardioangiography of radial nuclein (NCT) within 24 hours. Results: The results demonstrated that there existed significant difference of LVEF measured by TVI compared with 2D and M methods (0. 37±0. 2 : 0.46±0. 12, P<0. 05, 0. 37±0. 2: 0. 47±0. 08, P<0. 05), but it was similar to that of NCT (0.37±0.2 :0.37±0.17, P>0. 05) .Although LVEF were overestimated by M、2D method, compared with NCT, the results of these method were correlated with that of NCT significantly and the best correlation was founded in TVI technique (r=0. 957). the 2D method took second place of correlation (r = 0. 834). M method was in the end (r=0. 752). Conclusion: The method of TVI is superior in measuring of LVEF for case with remarkable enlargement of left ventricle.
