2种方法治疗儿童外伤松动前牙临床研究

Investigation of two methods in the treatment of loosen anterior teeth caused by trauma in children

目的探讨全牙列牙合垫和超强纤维树脂固定外伤松动前牙的临床效果。方法前牙外伤患儿38例,随机分为2组,观察组20例采用全牙列牙合垫固定治疗,对照组18例采用超强纤维树脂固定治疗。4周后拆除固定装置,统计2组有效固定率,比较2组牙龈探诊出血(bleeding on probing,BOP)阳性率、舒适指数和操作时间。结果观察组有效固定率(95.0%)与对照组(94.4%)比较差异无统计学意义(P>0.05);治疗前观察组BOP阳性率(45.0%)与对照组(27.8%)比较差异无统计学意义(P>0.05);治疗2、4周,观察组BOP阳性率(35.0%,15.0%)低于对照组(66.7%,94.4%),差异有统计学意义(P<0.05);观察组操作时间[220.0(200.0,240.0)s]、舒适指数(1.900±1.021)低于对照组[1 077.5(1 020.0,1 090.0)s,5.830±0.857](P<0.01)。结论全牙列牙合垫固定和超强纤维树脂固定治疗儿童外伤松动前牙均可达到有效固定,但全牙列牙合垫固定治疗的操作时间、舒适度及治疗后患儿牙周健康程度优于超强纤维树脂固定。

Objective To investigate the clinical effect of occlusal palate and super fiber resin fixation on the loosen anterior teeth caused by trauma. Methods Thirty-eight children with anterior teeth trauma were randomly divided into two groups. Observation group （n= 20） was treated with occlusal palate, and control group （n= 18） was treated with super fiber resin fixation. The effective fixation rate, bleeding on probing （BOP）, comfort index and operation lasting time were compared between two groups 4 weeks after removal of fixing device. Results There was no significant difference either in the effective fixation rate between observation group （95. 0%） and control group （94. 4%） （P〉 0.05）, or in the positive rate of BOP between observation group （45.0%） and control group （27.8%） before treatment （P〉0.05）. The positive rates of BOP after 2- and 4-week treatment in observation group （35. 0%, 15. 0%） were significantly lower than those in control group （66.7%, 94.4%） （P（0.05）. The operation lasting time （222.0（200.0, 240.0） s）, comfort index （1. 900 ±1. 021） in observation group were significantly lower than those in control group （1 077.5（1 020.0, 1 090.0） s, 5. 830± 0. 857） （P（0.01）. Conclusion Both two methods are effective for loosen anterior teeth caused by trauma in children. Oeclusal palate is superior to super fiber resin fixation in the operation lasting time, comfort degree and the degree of periodontal health after treatment.
