缺血性脑卒中患者血清FT_3、FT_4、TSH水平变化的临床意义 被引量:7

摘要 目的探讨缺血性脑卒中患者血清游离三碘甲状腺原氨酸(FT3)、血清游离甲状腺素(FT4)及促甲状腺激素(TSH)水平变化的临床意义。方法选取本院2014年1月~2015年1月收治的86例缺血性脑卒中患者作为研究组,选取同期的40例健康体检者作为对照组。记录两组的血清FT3、FT4、TSH水平,比较研究组不同神经功能缺损、预后患者的血清FT3、FT4、TSH水平。结果研究组治疗前的血清FT3、TSH水平显著低于治疗后与对照组,差异有统计学意义(P〈0.01)。随着NIHSS评分的增加,研究组的血清FT3水平呈降低趋势,组间比较差异有统计学意义(P〈0.05)。研究组的血清FT3水平与NIHSS评分呈负相关(r=-0.816,P〈0.05)。预后良好组的血清FT3、TSH水平显著高于预后不良组,差异有统计学意义(P〈0.01)。结论测定血清FT3、FT4、TSH水平有利于患者病情和近期预后的判断,在缺血性脑卒中的诊治中具有重要意义。 Objective To explore the clinical meaning of serum FT3,FT4 and TSH level changes in patients with cerebral ischemic stroke. Methods 86 patients with cerebral ischemic stroke from January 2014 to January 2015 in our hospital were selected as the study group,40 healthy persons during the same period were selected as the control group.The level of serum FT3,FT4 and TSH in the two groups was recorded respectively.The level of serum FT3,FT4 and TSH in the patients with different neurological function and prognosis in the study group was compared. Results The level of serum FT3 and TSH before treatment in the study group was lower than that before treatment and in the control group,with significant difference(P 〈0.01).With the increase of NIHSS score,the level of serum FT3 in the study group had decreased tendency,with significant difference(P 〈0.05).The level of serum FT3 in the study group had negative correlated with NIHSS score(r=-0.816,P〈0.05).The level of serum FT3 and TSH in the favorable prognosis group was higher than that in the adverse outcome group,with significant difference(P 〈0.01). Conclusion The testing of serum FT3,FT4 and TSH level is helpful for judgment patient condition and early prognosis,which has important meaning in the diagnosis and treatment of cerebral ischemic stroke.
