微型钛质钢板治疗锁骨中段粉碎性骨折的临床效果

Clinical effect of miniature titanium plate in treatment of comminuted fracture in the middle piece of clavicle

目的探讨微型钛质钢板治疗锁骨中段粉碎性骨折的临床效果。方法选取2014年4月~2015年1月我院收治的锁骨中段粉碎性骨折的56例患者进行研究,按照治疗方法将其分为对照组（n=25）和观察组（n=31）,对照组患者采取克氏针内固定治疗,观察组患者采取微型钛质钢板治疗,比较两组的临床疗效。结果对照组的优良率为72%,低于观察组的94%,观察组患者的术中出血量为（218.41±21.38）ml,明显少于对照组的（394.59±20.72）ml,手术时间为（91.49±15.51）min,明显短于对照组的（125.56±18.59）min,两组差异有统计学意义（P〈0.05）。观察组患者术后骨折愈合时间为（10.31±2.78）周、住院时间为（2.34±1.61）d,明显短于对照组的（16.68±3.35）周、（5.29±1.37）d,两组差异有统计学意义（P〈0.05）。观察组患者术后迟缓愈合、畸形愈合、固定松动、感染发生率明显低于对照组,两组差异有统计学意义（P〈0.05）。结论微型钛质钢板治疗锁骨中段粉碎性骨折的效果良好,能缩短手术时间和住院时间,有助于患者早日康复。

Objective To explore the clinical effect of miniature titanium plate in the treatment of comminuted fracture in the middle piece of clavicle. Methods From April 2014 to January 2015,56 patients with comminuted fracture in the middle piece of clavicle admitted into our hospital were selected.According to therapeutic method,they were divided into control group（n=25） and observation group（n=31）.In the control group,patients were treated for internal fixation by Kirschner wire,while in the observation group,mini-titanium plate for treatment was selected.The clinical effect between two groups was compared. Results The excellent and good rate in control group（72%） was lower than that in observation group accounting（94%）.In observation group,intraoperative amount of bleeding [（218.41 ±21.38） ml] was obviously less than that of control group [（394.59±20.72） ml],and operation time [（91.49±15.51） min] was obviously shorter than that of control group [（125.56±18.59） min],and there was a statistical difference between two groups（P〈0.05）.Healing time of postoperative fracture [（10.31±2.78） weeks] and hospital stay [（2.34±1.61） d] was obviously shorter than that of control group [（16.68±3.35） weeks and（5.29±1.37） d] respectively,and there was a statistical difference between two groups（P〈0.05）.The incidence rate of postoperative delayed heal,malunion,fixation loosening,and infection in observation group was lower than that of control group,and there was a statistical difference between two groups（P 〈0.05）.Conclusion Miniature titanium plate in treatment of comminuted fracture in the middle piece of clavicle can obtain a favorable effect.It can shorten operation time and hospital stay,and is helpful for patients to recover soon.
