孕妇妊娠晚期B族链球菌感染对妊娠结局及新生儿的影响

Influence of group B Streptococcus infection in late trimester of pregnancy on pregnancy outcome and newborns

目的研究孕妇妊娠晚期B族链球菌（GBS）感染对妊娠结局及新生儿的影响。方法选取2015年1~7月来我院进行产检的920名孕妇作为研究对象,将GBS感染阳性孕妇作为阳性组（76例）,GBS感染阴性孕妇作为阴性组（844例）,分析GBS感染对妊娠结局与新生儿的影响。结果 GBS阳性组发生宫内感染13例（17.11%）,胎儿窘迫23例（30.26%）,羊水污染13例（17.11%）,早产17例（22.37%）,产后出血9例（11.84%）,以上各发生率显著高于阴性组,两组差异有统计学意义（P〈0.05）。GBS阳性组新生儿出现新生儿肺炎5例（6.58%）,病理性黄疸12例（15.79%）,新生儿窒息9例（11.84%）,以上各发生率显著高于阴性组,两组差异有统计学意义（P〈0.05）。结论孕妇妊娠晚期出现GBS感染会引起宫内感染、胎膜早破与新生儿感染等不良结局,临床中应高度重视妊娠晚期GBS感染孕妇,并对其行早期干预治疗,以改善妊娠结局。

Objective To study the influence of group B Streptococcus（GBS） infection in late trimester of pregnancy on pregnancy outcome and newborns. Methods 920 pregnant women receiving antenatal care in our hospital from January to July 2015 were selected,and GBS positive patients were divided into the positive group（76 cases）,while GBS negative patients were divided into the negative group（844 cases）.The influence of GBS infection on pregnancy outcome and newborns were explored. Results In GBS positive group,of which 13（17.11%） cases experienced intrauterine infection,23 cases（30.26%） experienced fetal distress,13 cases（17.11%） experienced amniotic fluid pollution,17 cases（22.37%）experienced premature delivery and 9 cases（11.84%） experienced postpartum hemorrhage.The incidences of the above events in positive group were significantly higher than those of negative group with statistical significance（P〈0.05）.In GBS positive group,5 cases（6.58%） experienced pneumonia of newborn,12 cases（15.79%） experienced pathologic jaundice and 9 cases（11.84%） experienced neonatal asphyxia.The incidences of the above events in positive group were significantly higher than those of negative group with statistical significance（P 〈0.05）. Conclusion GBS infection in late trimester of pregnancy can lead to adverse consequences such as intrauterine infection premature rupture of fetal membranes and neonatal infection.Therefore,high attention should be paid to patients with GBS infection in late trimester of pregnancy,and intervention treatment should be conducted as early as possible to improve the pregnancy outcomes.
