营养对重症急性胰腺炎患者肠源性感染的影响研究 被引量:14

Impact of nutrition on intestinal infections in patients with severe acute pancreatitis

摘要 目的探究肠内营养(EN)与肠外营养(PN)对重症急性胰腺炎肠源性感染的临床影响程度,为临床治疗提供数据参考。方法选取2014年1月-2015年8月110例重症急性胰腺炎肠源性感染患者,按照不同的营养方式将患者分为试验组与对照组各55例;两组患者均给予常规治疗,观察组给予EN治疗,对照组给予PN治疗,对比分析两组患者肠源性感染指标的变化,采用SPSS12.0软件进行统计数据分析。结果患者治愈率观察组为94.55%、对照组为85.45%,两组比较差异有统计学意义(P<0.05);不良反应发生率观察组为12.73%、对照组为40.00%,两组比较差异有统计学意义(P<0.05);EN与PN在血白细胞、内毒素以及C-反应蛋白等方面的临床指标比较差异有统计学意义(P<0.05)。结论重症急性胰腺炎肠源性感染患者早期实施EN的治疗方式,治疗效果最理想;EN治疗方式可有效提高治愈率,降低不良反应发生率,从而减轻重症急性胰腺炎肠源性感染患者的痛苦,值得临床推广。 OBJECTIVE To observe the clinical impact of enteral nutrition(EN)and parenteral nutrition(PN)on the intestinal infections in the patients with severe acute pancreatitis so as to provide guidance for clinical treatment.METHODS A total of 110 severe acute pancreatitis patients complicated with intestinal infections who were treated in the hospital from Jan 2014 to Aug 2015 were enrolled in the study and divided into the experimental group and the control group according to the nutrition support mode,with 55 cases in each.Both groups were given the conventional therapy,the observation group was treated with EN,and the control group was treated with PN.The changes of the indicators of intestinal infections were observed and compared between the two groups of patients,and the statistical analysis of data was performed with the use of SPSS12.0software.RESULTS The cure rate was 94.55%in the observation group,85.45%in the control group,and there was significant difference between the two groups(P〈0.05).The incidence of adverse reactions was 12.73% in the observation group,40.00%in the control group,and there was significant difference between the two groups(P〈0.05).There was significant difference in such clinical indicators as the white blood cells,endotoxin,and C-reactive protein between the patients treated with EN and the patients treated with PN(P〈0.05).CONCLUSION The implementation of EN in early stage can achieve the most desirable therapeutic effect on treatment of the severe acute pancreatitis patients with intestinal infections.The EN treatment approach can effectively raise the cure rate and reduce the incidence of adverse reactions so as to alleviate the sufferings of the severe acute pancreatitis patients with intestinal infections,and it is worthy to be promoted in the hospital.
