摘要
目的探讨运用多准则决策分析法构建集束干预策略对膝关节置换患者术后感染的预防效果。方法将2011年1月-2012年12月行膝关节置换术的1 138例患者作为对照组,2013年1月-2014年12月行膝关节置换术的1 362例患者作为观察组,运用多准则决策分析法构建集束化干预策略并将其运用于患者的护理中,比较两组患者术后感染的发生情况;采用SPSS17.0软件对数据进行统计分析。结果观察组1 362例行膝关节置换术患者中15例发生感染,感染率为1.10%,检出病原菌17株,对照组1 138例行膝关节置换术患者中26例发生感染,感染率为2.28%,检出病原菌34株,两组差异有统计学意义(P<0.05);观察组患者膝关节HSS评分、住院天数分别为(83.4±5.7)分、(14.8±3.6)d,均优于对照组的(81.9±6.8)分、(15.7±4.1)d,两组差异均有统计学意义(P<0.05)。结论将采用多准则决策分析法构建的集束干预策略应用于行膝关节置换术的患者,能有效减少病原菌的侵入、降低术后感染的发生率。
OBJECTIVE To establish the bundle intervention strategy by means of multi-criteria decision analysis method so as to prevent the postoperative infections in the patients undergoing knee arthroplasty.METHODS A total of 1 138 patients who underwent the knee arthroplasty from Jan 2011 to Dec 2012 were chosen as the control group,and 1 362 patients who underwent the knee arthroplasty from Jan 2013 to Dec 2014 were set as the observation group.The bundle intervention strategy was established by using multi-criteria decision analysis method and was used for the care of patients,the incidence of postoperative infections was observed and compared between the two groups of patients,and the statistical analysis of data was performed with the use of SPSS17.0software.RESULTS Of 1 362 patients undergoing the knee arthroplasty in the observation group,15 had the infections,with the infection rate of 1.10%,and 17 strains of pathogens were isolated.Of 1 138 patients undergoing the knee arthroplasty in the control group,26 had the infections,with the infection rate of 2.28%,and 34 strains of pathogens were isolated;there was significant difference between the two groups(P〈0.05).The HSS score of the observation group was(83.4±5.7)points,higher than(81.9±6.8)points of the control group;the average hospitalization duration of the observation group was(14.8±3.6)days,shorter than(15.7±4.1)days of the control group,and there was significant difference between the two groups(P〈0.05).CONCLUSION The bundle intervention strategy established based on the multi-criteria decision analysis method can effectively prevent the invasion of pathogens and reduce the incidence of postoperative infections in the patients undergoing the knee arthroplasty.
