ICU综合疗法治疗重症胰腺炎的效果观察

Effect observation of ICU comprehensive therapy in the treatment of severe acute pancreatitis

目的探讨ICU综合疗法治疗重症胰腺炎的效果。方法选取本院2012年1月~2014年5月收治的58例重症胰腺炎患者作为研究对象,根据治疗处置方法分为A组（31例）和B组（27例）。B组采用常规治疗方案,A组给予ICU综合疗法,在常规治疗的基础上给予血液净化治疗,比较两组的疗效。结果 A组的总有效率为93.5%,显著高于B组的77.8%,差异有统计学意义（P〈0.05）。A组的腹痛缓解时间、腹胀缓解时间、恶心呕吐缓解时间、排气排便恢复时间、血与尿淀粉酶恢复时间显著短于B组,差异有统计学意义（P〈0.05）。两组治疗后的血、尿淀粉酶水平显著低于治疗前,差异有统计学意义（P〈0.01）。A组治疗后的血、尿淀粉酶水平显著低于B组,差异有统计学意义（P〈0.05）。血液净化治疗12 h与治疗结束时的PaO_2与SaO_2水平显著高于治疗前,APACHEⅡ评分显著低于治疗前,差异有统计学意义（P〈0.05）。结论在常规治疗的基础上进行血液净化治疗可有效改善重症胰腺炎患者的临床症状,恢复胰腺功能,疗效确切。

Objective To explore the effect of ICU comprehensive therapy in the treatment of severe acute pancreatitis.Methods 58 cases of severe acute pancreatitis from January 2012 to May 2014 in our hospital were selected and divided into group A（31 cases）and group B（27 cases）according to treatment and disposal method.Group B was treated with routine treatment method,group A was treated with ICU comprehensive therapy,static blood therapy was given on the basis of the routine treatment.The efficacy of the two groups was compared.Results The total effective rate of group A was 93.5%,which was higher than 77.8% of group B,with significant difference（P0.05）.Abdominal pain relief time,abdominal distension relief time,nausea and vomiting relief time,exhaust defecation recovery time,blood and urine amylase recovery time in group A was shorter than that group B,with significant difference（P0.05）.The level of blood and urine amylase after treatment in two groups was lower than that before treatment,with significant difference（P0.01）.The level of blood and urine amylase in group A was lower than that group B,with significant difference（P 0.05）.The level of PaO_2 and SaO_2 in the treatment of 12 h and in the end of static blood therapy was higher than that before treatment,the score of APACHE Ⅱ in the treatment of 12 h and in the end of static blood therapy was lower than that before treatment,with significant difference（P 0.05）.Conclusion Blood purification therapy can effectively improve the clinical symptom of severe pancreatitis patients on the basis of the routine treatment,recovery of pancreatic function,curative effect is exact.
