比索洛尔联合依达拉奉对PCI术后再灌注心律失常的预防及抗氧化作用分析

Prevention and antioxidant role analysis of bisoprolol combined with edaravone for reperfusion arrhythmia after PCI operation

目的探讨比索洛尔联合依达拉奉对PCI术后再灌注心律失常的预防效果及抗氧化作用。方法选择2014年1~12月本院拟行急诊经皮冠脉介入（PCI）的急性心肌梗死患者90例,采用随机数字表法分为两组,每组45例。两组术前均行常规PCI准备,对照组患者术前给予比索洛尔口服预防再灌注心律失常,研究组患者给予依达拉奉静脉输注联合比索洛尔口服预防再灌注心律失常。全部患者术中、术后24 h给予持续心电监护,且术前、术后24 h检查血清超氧化物歧化酶（SOD）、丙二醛（MDA）、肌钙蛋白T（c Tn T）变化情况。结果两组患者再灌注心律失常的主要类型是室性期前收缩和阵发性室性心动过速,其中研究组的发生率明显低于对照组,差异有统计学意义（P〈0.05）。治疗后,研究组的血清SOD水平上升值明显高于对照组,MDA水平下降值明显高于对照组,c Tn T水平上升值明显低于对照组,两组比较,差异有统计学意义（P〈0.05）。结论比索洛尔联合依达拉奉能有效预防PCI术后再灌注心律失常,且具有显著的抗氧化作用,可保护缺血心肌,值得应用。

Objective To explore the prevention effect and antioxidant role of bisoprolol combined with edaravone for reperfusion arrhythmia after percutaneous coronary intervention（PCI）operation.Methods 90 patients with acute myocardial infarction（AMI）scheduled for emergency PCI from January to December 2014 in our hospital were selected,and they were divided into two groups according to the random number table method,and there were 45 cases in each group.Two groups were given routine preoperative preparation.In order to prevent reperfusion arrhythmia,patients in control group were given bisoprolol（oral administration）,while patients in research group were given edaravone（intravenous infusion）combined with bisoprolol（oral administration）.All patients were given intraoperative and postoperative24 h continuous ECG monitoring,Before operation and after 24 h operation,serum superoxide dismutase（SOD）,malondialdehyde（MDA）,troponin T（c Tn T）change was detected respectively.Results The main type of reperfusion arrhythmia in two groups was ventricular premature contraction and paroxysmal ventricular tachycardia respectively,and the incidence rate of research group was significantly lower than that in control group,the difference was statistically significant（P0.05）.The rise value of serum SOD level in research group was obviously higher than that of control group,the drop value of MDA level was significantly higher than that of control group,rise value of c Tn T level was significantly lower than that of control group,and there was a statistical difference between two groups（P 0.05）.Conclusion Bisoprolol combined with edaravone can effectively prevent the reperfusion arrhythmia after PCI operation,and has a significant antioxidant role,can protect ischemic myocardium,and it is worthy of application.
