Baseline investigation of stroke cohort in rural area of Shanghai city

摘要 目的总结报告上海中老年人群脑卒中队列基线调查资料。方法以现况研究设计、整群抽样的方法 ,抽取上海市长宁区程桥街道部分社区≥40岁非体力劳动人群,进行一般情况、常见脑卒中危险因素调查和血液动力学指标检测,并以统一的方法对血液动力学指标进行积分。选取符合纳入标准、资料完整的人群共计5 335例,分析比较危险因素暴露水平、脑卒中患病率、血液动力学检测指标,并描述其分布特征。结果男性高血压、心脏病、糖尿病、高血压病家族史、脑卒中家族史、超重或肥胖等危险因素的暴露率分别为31.74%、6.09%、1.16%、3.22%、17.64%和29.68%,女性分别为32.76%、9.22%、1.55%、3.84%、19.22%和29.44%;脑卒中标准化患病率为1 167.3/10万,男性脑卒中患病率显著高于女性(P<0.05);血液动力学检测指标变化与年龄密切相关,积分<75分者占21.5%。结论城镇社区≥40岁人群脑卒中危险因素中高血压暴露率最高,脑卒中患病率男性高于女性,约20%脑血管血液动力学异常。 Objective To summarize data from a baseline investigation of stroke cohort in rural area of Shanghai city. Methods A cross-sectional study was carried out in a duster sampling population aged 50 years and over. General information, common risk factors in the population were investigated and cerebral vascular hemodynamie indexes were examined. Hemodynamie score was estimated according to single indexes by unified methods. Finally 5335 persons meet the inclusion criteria were enlisted in the study. Exposure level of risk factors, prevalence of stroke,and hemodynamie indexes were analyzed and distributional characteristics were described. Results Exposure rate of hypertension, heart disease, diabetes, family history of hypertension, overweight or obesity in male were 31.74% ,6.09%, 1.16% ,3.22%, 17.64% ,29.68% ,and in female were 32.76% ,9.22%, 1.55%, 3.84%, 19.22%, 29.44%,respectively. Standard prevalence of stroke was 1167.3/100 000,whieh in male was significantly higher than that in female (P 〈 0.05). Change of cerebral vascular hemodynamie indexes was significantly associated with age. Hemodynamie score in 21.48 percent subjects was below 75 points. Conclusion Among rural area of 40 years old and over population, hypertension is the highest exposure rate risk factors of stroke. Prevalence of stroke in male is significantly higher than that in female. Abnormal rate of hemodynamie score is about one fifth.
