IVIG治疗儿童重症腺病毒肺炎的临床疗效和并发症观察

Clinical effect and complications of IVIG in the treatment of children with severe adenovirus pneumonia

目的分析静脉用丙种球蛋白（IVIG）应用于儿童重症腺病毒肺炎（SAP）的疗效及安全性。方法选择180例重症腺病毒肺炎儿童的临床资料进行回顾性分析,依据是否给予IVIG治疗分为观察组与对照1组,对两组患者治疗后的临床疗效、住院时间、发热持续时间、机械通气时间及并发症的发生情况进行对比。结果与对照组对比,观察组患儿的发热时间、住院时间及机械通气时间均显著缩短,痊愈率与好转率均升高,差异有统计学意义（P〈0.05）。其中在无效的人数中,观察组死亡3例（3.03%）,对照组死亡4例（4.94%）,两组对比差异无统计学意义（χ^2=0.01,P=0.732）;与对照组对比,观察组患儿发生胸腔积液、肺不张、中毒性脑病并发症的发生率显著降低,差异有统计学意义（P〈0.05）,两组患儿呼吸衰竭、肺功能损害的发生率差异无统计学意义（P〉0.05）。结论 IVIG治疗儿童SAP有一定的疗效,能降低并发症。

Objective To analyze the clinical effect and complications of intravenous vein gamma globulin（ IVIG） in the treatment of children with severe adenovirus pneumonia（ SAP）. Methods The clinical data of 180 children with SAP were retrospectively analyzed,and they were divided into the study group and the control group according to whether IVIG treatment was applied or not. Their clinical efficacy,duration of hospital stay,fever continued time,mechanical ventilation time and complications were compared. Results Compared with the control group,their fever time,duration of hospital stay and mechanical ventilation time were significantly shorter in the study group（ P〈0. 05）. The curative rate and improved rate were better in the study group than in the control group. There were 3 cases of death in the study group and 4 cases of death in the control group（ χ^2= 0. 01,P = 0. 732）. The incidence of pleural effusion,atelectasis and toxic encephalopathy was obviously lower in the study group than in the control group（ P〈0. 05）. There was no significant difference in respiratory failure and pulmonary function damage between the two groups（ P〉0. 05）. Conclusion IVIG has good curative effect in the treatment of children with SAP,with low complications.
