目的:探讨运用PDCA循环降低住院患者口服药非安全发放的效果。方法:采用PDCA循环4个阶段、8个步骤对我院内科10个病区住院患者口服药非安全发放较多进行原因分析,运用鱼骨图分析法找出"医嘱录入与执行中存在缺陷""与服药相关硬软件不完善"是住院患者口服药非安全发放较多的主要原因,针对要因制定、实施对策,检讨和效果确认,对于效果明显的措施实行标准化。结果:内科10个病区住院患者口服用药非安全发放改善前与改善后相比差异有统计学意义(P<0.05)。结论:PDCA循环能够充分了解住院患者口服药非安全发放较多的原因,从而制定了有针对性、护士能力范围的措施,明显降低了内科10个病区住院患者口服药非安全发放例数,提高了患者遵医服药的依从性及口服用药知识的知晓度,从而达到知情同意、主动配合、有效治疗的目的,确保患者口服用药的安全性。

Objective：To explore the effect of PDCA circulation on reducing the non - safety deliver of oral medicines for inpatients. Methods：PDCA circula- tion were used to analyze the reasons for the large amount of non - safety deliver of oral medicines for inpatients in the 10 wards in the internal medicine in our hospital,the fishbone diagram analysis was used to find out that the ＂defects in the entry and implementation of the doctors＇ advice＂ and ＂imperfections in the hardware and software related to medicine＂ were the main reasons for the large amount of non - safety deliver of oral medicines for inpatients. We have made and implemented countermeasures for the main reasons, made reviews and confirmed the effects, and applied standardization for the measures with ob- vious effects. Results：The differences before and after the improvement in the non - safety deliver of oral medicines for inpatients in the 10 wards in the in- ternal medicine were statistically significant （ P 〈 0.05 ）. Conclusion ： PDCA circulation can fully understand the reasons for the large amount of non - safety deliver of oral medicines for inpatients, so we can make targeted measures within the abilities of nurses. These significantly reduce the number of non - safety deliver of oral medicines for inpatients in the 10 wards in the internal medicine and improve the compliance of taking medicine according to the doctors＇ ad- vice and awareness of oral medication knowledge. In this way, the patients know more, would like to coordinate with the doctor, and their diseases could ef- fectively cure, the safety of oral medicine could also ensure.
