CRP、sTREM-1、TNF-α在小儿支原体肺炎中的临床价值分析

Analysis on clinical value of CRP,sTREM-1 and TNF-αin children with Mycoplasma pneumoniae pneumonia

目的探讨血清C反应蛋白(CRP)、可溶性髓样细胞触发受体-1(sTREM-1)、肿瘤细胞坏死因子-α(TNF-α)在小儿支原体肺炎(MPP)中的临床应用价值。方法 2014年1月至2014年12月选取本院120例MPP患儿为研究对象(MP组),另选取同期收治的100例健康体检儿童为对照组,采用免疫比浊法测定两组CRP水平,采用ELISA法测定两组血清sTREM-1、TNF-α水平。结果 MPP组患儿血清CRP、sTREM-1、TNF-α水平显著高于对照组(P<0.05)。与治疗第1天相比,MPP组患儿治疗第4天、7天血清CRP、sTREM-1、TNF-α水平显著下降,差异有统计学意义(P<0.05)。存活组患儿血清CRP、sTREM-1、TNF-α水平低于死亡组(P<0.05)。经ROC曲线显示,CRP、sTREM-1、TNF-α在MPP中诊灵敏度依次为78.3%、85.0%和92.5%,特异性依从性为76.0%、90.0%和89.0%。结论血清CRP、sTREM-1、TNF-α在MPP中的诊断中具有一定的诊断价值,可作为MPP病情进展及预后的评价指标。

Objective To investigate the serum C-reactive protein（CRP）,soluble myeloid cells trigger（sTREM-1）,tumor necrosis factor-α（TNF-α）receptor-1in clinical pediatric mycoplasma pneumonia（MPP）in application.Methods The levels of CRP of120 cases of children with mycoplasma pneumonia and 100 cases of children in the control group of normal physical examination were measured with immunonephelometry,the levels of sTREM-1,TNF-αof two groups were determined by ELISA method from January 2014 to December 2014.Results The levels of CRP,sTREM-1,TNF-αof MPP group were higher than control group（P〈0.05）.The levels of CRP,sTREM-1,TNF-αafter of MPP groups after 4d,7dwere lower than 1d（P〈0.05）.The levels of CRP,sTREM-1,TNF-αof survivors group were lower than the death group（P〈0.05）.The sensitivity and sequentially of CRP,sTREM-1,TNF-αof MPP groups were 78.3%,85.0%,92.5% and 76.0%,90.0%,89.0%.Conclusion Serum CRP,sTREM-1,TNF-αdiagnosis of mycoplasma pneumonia in children has certain diagnostic value as Mycoplasma Pneumonia evaluation of disease progression and prognosis.
