丹鳖汤治疗盆腔炎湿热瘀阻证临床研究

The Clinical Research into Pelvic Inflammatory Disease with Syndrome of Damp and Heat Congestion Treated with Dan Bie Decoction

目的:观察丹鳖汤治疗盆腔炎湿热瘀阻证的临床疗效。方法:选择2012年4月—2014年8月就诊于本院的96例湿热瘀阻型盆腔炎患者为研究对象,随机分为对照组和观察组各48例,对照组给予甲硝唑注射液联合左氧氟沙星注射液治疗,观察组在对照组治疗的基础上给予丹鳖汤配合坤复康胶囊治疗,两组患者均以1周为1个疗程,连续治疗2个疗程。结果:两组患者经治疗后盆腔包块直径较治疗前有所缩小,观察组患者的缩小程度较对照组患者更明显,差异有统计学意义(P<0.05);两组患者经治疗后机体炎性细胞因子水平较治疗前有所改善,白细胞计数(WBC)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)水平均较治疗前有所下降,观察组的下降幅度明显大于对照组,差异有统计学意义(P<0.05);两组患者治疗后血液流变学指标改善明显,观察组患者各指标的改善程度较对照组患者更明显,差异有统计学意义(P<0.05);对照组有效率为77.1%,明显低于观察组的有效率93.8%,差异有统计学意义(P<0.05)。结论:丹鳖汤配合坤复康胶囊能有效缩小湿热瘀阻型盆腔包块的直径,降低炎性因子水平,改善患者血液循环,疗效显著。

Objective： To observe the clinical curative effect of Dan Bie Decoction（ DBD） on pelvic inflammatory disease with syndrome of damp and heat congestion. Methods： Ninety-six patients with pelvic inflammatory disease with syndrome of damp and heat congestion who were treated in our hospital between April of 2012 and August of 2014 were chosen as the research objects and randomly divided into the control group and the observation group,with 48 cases in each group. The ones in the control group were given metronidazole injection and levofloxacin injection for treatment,while the ones in the observation group were added DBD in combination with Kun Fu Kang Capsule（ KFKC） based on the treatment of the control group. Both the two groups regarded one week as one treatment course,and the patients would be treated continuously for 2 courses. Results： After the treatment,the pelvic bag piece diameter of the two groups was narrowed than before the treatment,and the narrowed degree of the observation group was more obviously than that of the control group,and the difference was statistically significant（ P〈0. 05）. After the treatment,the body＇s inflammatory cell factor level of the two groups was improved than before the treatment,WBC,CRP,IL-6 and TNF-α levels were decreased; The decreased degree of the observation group was more obviously than that of the control group,and the differences were statistically significant（ P〈0. 05）. The hemorheology index of the two groups was improved significantly after the treatment,and the improved degree of each index of the observation group was more significantly than that of the control group（ P〈0. 05）. The effective rate of the control group was 77. 1%,which was much lower than 93. 8% of the observation group（ P〈0. 05）. Conclusion： DBD in combination with KFKC has the remarkable clinical curative effect,can effectively narrow the diameter of pelvic inflammatory disease with syndrome of damp and heat congestion,reduce inflammatory cell factor level and improve the patients＇ blood circulation.
