摘要 目的:分析3个T细胞亚群,CD4+T、CD8+T和调节性T(regulatory T,Treg)细胞在胰腺癌病人外周血中的变化,为胰腺癌免疫治疗中靶细胞的确认提供理论依据。方法:通过免疫荧光抗体标记和流式细胞检测外周血中CD4+T、CD8+T和Treg细胞,分析胰腺癌病人外周血中CD4+T、CD8+T和Treg细胞的水平与胰腺癌临床病理特征的相关性。结果:胰腺癌病人外周血中CD4+T、CD8+T的水平与对照组相比无统计学差异,而Treg细胞在CD4+T细胞中的比例较对照组显著升高;除了Treg的比例与胰腺癌的疾病分期密切相关外,3个细胞亚群的水平与胰腺癌病人临床病理特征无相关性。胰腺癌合并梗阻性黄疸行胆道支架引流术后,CD4+T、CD8+T细胞的水平无变化,但Treg细胞的比例显著下降。结论:在胰腺癌病人的免疫功能状态的变化中Treg细胞介导的免疫抑制可能占主导地位。合并梗阻性黄疸行胆道引流术可缓解Treg细胞介导的免疫抑制。 Objective To analyze the change in amount of CD4+T, CD8+T, regulatory T cells (CD4+CD25+Foxp3+, Treg) in peripheral blood from pancreatic cancer patients and to provide the basis in immunotherapy for pancreatic cancer. Methods The amount of CD4+T, CD8+T, Treg cells in peripheral blood from pancreatic cancer patients were detected using the immune fluorescent antibody and flow cytometry. The association of CD4+T, CD8+T, Treg cells with clinicopathological aspects was analyzed. Results There was no significant difference of amount of CD4+T, CD8+T in peripheral blood between pancreatic cancer group and control group without tumor. The ratio of Treg cells to CD4+T was significantly higher in pancreatic cancer group than that in control group. The amount of CD4+T, CD8+T, Treg cells had no correlation with clinicopathological aspects of pancreatic cancer, while the ratio of Treg to CD4+T significantly correlated with the pathological stage of pancreatic cancer. There was no significant change of the amount of CD4+T, CD8+T in pancreatic cancer patients after bile duct drainage while the ratio of Treg cells to CD4+T decreased dramatically. Conclusions The immunosuppression mediated by Treg cells dominated immune status among pancreatic cancer patients. The bile duct drainage in pancreatic cancer patients with obstructive jaundice could alleviate the immunosuppression mediated by Treg cells.
