摘要
目的探讨癌因性疲乏(CRF)肺癌患者采用集束化干预策略的效果。方法将2014年2月至2015年3月中国石油天然气集团公司中心医院明确诊断为肺癌且未接受化疗的87例CRF肺癌患者作为研究对象,依据随机数字表法分为试验组(43例)和对照组(44例)。对照组给予镇痛、止咳、化痰健康教育等常规治疗和干预,试验组在常规治疗基础上给予集束化干预策略,时间为第1次化疗前3 d至第3次化疗前,持续时间为2个月。于干预前、第3次化疗前使用Piper疲乏修订量表(PES-R)评估不同干预措施后肺癌患者CRF状况。结果 2个月干预结束,与对照组患者相比,试验组患者CRF程度明显减轻,差异有统计学意义(P<0.05)。试验组患者CRF总分为及行为疲乏、情感疲乏、感觉疲乏、认知疲乏4个维度均较对照组低[(4.2±1.2)分比(5.4±1.7)分,(4.1±1.4)分比(5.4±1.6)分,(3.5±0.6)分比(5.0±1.5)分,(4.0±1.1)分比(5.0±1.6)分,(4.3±1.2)分比(5.6±1.3)分],差异有统计学意义(P<0.05)。结论集束化干预策略应用于化疗的肺癌患者,能有效减轻肺癌患者的CRF,提高患者的生活质量。
Objective To explore the effect of cluster intervention strategies on lung cancer patients with cancer-related fatigue( CRF). Methods We recruited 87 patients who were diagnosed with lung cancer with CRF before chemotherapy in China National Petroleum Corporation Center Hospital form Feb. 2014 to Mar.2015. According to random number table method they were divided into trial group( 43 cases) and control group( 44 cases). The control group only received routine therapy and care,for example,pain relief,cough and phlegm relief,and health education,and the trial group was added cluster intervention strategies on the basis of conventional treatment,from the three days before chemotherapy to before the third time chemotherapy,for2 months. Revised Piper fatigue scale( PES-R) was used to evaluate the CRF condition of the two groups.Results After the 2 months of intervention,compared with the control group,the CRF of the trial group was significantly reduced( P〈0. 05). The total fatigue scores,behavior fatigue,emotional fatigue,feeling fatigue,cognitive fatigue of the trial group were lower than the control group [( 4. 2 ± 1. 2) scores vs( 5. 4 ±1. 7) scores,( 4. 1 ± 1. 4) scores vs( 5. 4 ± 1. 6) scores,( 3. 5 ± 0. 6) scores vs( 5. 0 ± 1. 5) scores,( 4. 0 ± 1. 1) scores vs( 5. 0 ± 1. 6) scores,( 4. 3 ± 1. 2) scores vs( 5. 6 ± 1. 3) scores]. Conclusion Cluster intervention strategies can alleviate CRF of lung cancer patients receiving chemotherapy and improve their quality of life.
