摘要
目的:关注静脉药物配置中心输液医嘱情况,促进临床合理用药。方法:对静脉药物配制中心2014年1月—2014年12月接收的415 969条输液医嘱进行审核,对不合理用药进行分析。结果:不合理用药医嘱共635条,占总医嘱数的0.15%,主要表现为药品配伍不当(13.54%)、药品剂量超说明书用药(20.00%)、药品溶媒选择不合理(39.84%)、药品浓度不合理(26.62%)。结论:岳阳市一人民医院病区输液用药存在一定的不合理现象,药学人员应多与临床医生沟通并加强对药物知识的学习,有利于控制不合理用药,提高合理用药水平。
Objective： To pay attention to the situation of drug in pharmacy intravenous admixture services（ PIVAS） and promote the proper medication in clinical. Methods： To reviewed 415 969 pieces of medicine prescriptions in PIVAS of hospital from January to December in 2014,and to analyze the irrational presdcriptions. Results： There were 635 irrational prescriptions,accounting for 0. 15%,including uncompatibility of the drugs（ 13. 54%）,dose exceeding the drug instructions（ 20. 00%）,unreasonable solvent of drug（ 39. 84%） and unreasonable drug concentration（ 26. 62%）. Conclusion： There were some unreasonable intravenous use in some wards in our hospital,the pharmacy staff should communicate with doctors and strengthen learning the knowledge of drugs,to help to control the irrational drug use and improve the level of rational use of drugs.
