彩色多普勒超声诊断心脏粘液瘤的临床价值研究

The clinical value of color Doppler ultrasound in the diagnosis of cardiac myxoma

目的对彩色多普勒超声诊断心脏粘液瘤的临床价值分析。方法2013年7月至2015年7月在我院实施诊断及治疗的32例心脏粘液瘤患者，均对其实施彩色多普勒超声诊断，对患者的临床检测结果实施回顾性分析。结果在32例患者临床诊断中，均发现附着在卵圆窝附近的蒂。其中16例患者的瘤体形状不够规则同时出现分叶，16例患者瘤体较为规则；14例患者瘤体较大，其中分别进入心室腔内、堵塞心室三尖瓣口、堵塞二尖瓣口。所有患者瘤体最大为7．6cm×5．3cm×3．6cm，最小为1．9cm×1．5cm×1．2cm。如果患者是左心房粘液瘤患者，则诊断中可以见杂音在心尖区，并且左侧卧位时杂音更为清晰；如果患者是右心房粘液瘤患者，则诊断中可以见杂音在胸骨左缘的3—4肋间，并且右侧卧位时杂音更为清晰。结论彩色多普勒超声在心脏粘液瘤患者临床诊断中，具有较高检出率，能够为患者临床治疗提供重要的参考资料，因此说彩色多普勒超声在心脏粘液瘤患者临床诊断中具有重要应用价值，值得推广应用。

Objective To analyze the clinical value of color Doppler ultrasound in the diagnosis of cardiac myxoma. Methods 32 cases of cardiac myxoma undergoing the diagnosis and treatment in our hospital from July 2013 and July 2015 were treated with color Doppler ultrasound, patients＇ clinical detection results were retrospectively analyzed. Results In the clinical diagnosis of 32 patients, pedicle was found attached to fossa ovalis in all patients. The shape of the tumor was not enough and the shape of the tumor was divided into 26 cases, and the tumor of 16 cases were more regular. 14 patients had large tumors, which entered into the ventricular cavity, blocked left ventricular tricuspid valve orifice and mitral valve orifice. The maximum size of the tumor was 7.6 cm×5.3 cm×3.6 cm, and the minimum size was 1.9 cm×1.5 cm×1.2 cm. In patients with left atrial myxoma, diagnosis showed murmur at the apical region of heart, murmur was more clear when left lateral decubitus position; in patients with right atrial myxoma, diagnosis showed murmur at the left sternal border of 3-4 intercostal, murmur is more clear when right lateral decubitus position. Conclusions Color Doppler ultrasound has high detection rate in the diagnosis of cardiac myxoma, can provide an important reference for the clinical treatment of patients. Color Doppler ultrasound has important application value in the diagnosis of cardiac myxoma, worthy of clinical popularization.
