摘要 目的探讨控制性低血压对脑电双频谱指数(BIS)监测的影响。方法择期拟在全身麻醉下行胸、腰椎骨折椎体减压、植骨内固定的患者25例,依术中要求采用硝普钠0.05~2μg/(kg·min)静脉泵注控制性降压,维持MAP至基础值的70%。记录麻醉前(T_0)、控制性降压前(T_1)、控制性降压后(T_2)、停用降压药物后MAP回升至控制性降压前水平(T_3)BIS值。结果 BIS值在T_0与T_1、T_1与T_2、T_2与T_3时间点分别比较,差异有统计学意义(P〈0.01);在T_1与T_3时间点比较,差异无统计学意义(P〉0.05),患者在T_2时间点上有6例监测出爆发-抑制波形。结论全身麻醉期间控制性低血压可使脑电双频谱指数降低,进而影响麻醉深度的判断。 Objective To investigate the effects of controlled hypotension on bispectral index( BIS) monitoring. Methods 25 patients of elective general anesthesia thoracic,lumbar vertebral decompression,internal fixation,underwent sodium nitroprusside 0. 05 ~ 2μg /( kg·min) intravenous infusion to induce hypotension and maintain MAP to 70% baseline values according to operation. The value of BTS were recorded before anesthesia( T0),before controlled hypotension( T1),controlled hypotension after( T2),disable after antihypertensive drugs MAP recovered to the level before controlled hypotension( T3). Results BIS values in T0 and T1,T1 and T2,T2 and T3 time points,respectively,the difference were statistically significant( P〈0. 01);compared the value of BTS at T3 time point with T1,the difference was not statistically significant( P〈0. 05) 。Patients at time point T2 had monitored six cases of the outbreak-suppression waveform. Conclusion Controlled hypotension during general anesthesia can decreased bispectral index,thereby affecting the judge the depth of anesthesia.
