不同病理类型卵泡膜纤维瘤组肿瘤的MRI特征表现 被引量:15

摘要 目的探讨不同病理类型卵泡膜纤维瘤组肿瘤的MRI表现特征。方法搜集18例经病理证实的不同类型卵泡膜纤维瘤组肿瘤患者的MRI资料。18例中,卵泡膜细胞瘤7例,卵泡膜纤维瘤6例,纤维瘤5例(其中1例为细胞型纤维瘤)。回顾性分析3种病理类型病灶的MRI信号、强化及退变等表现,比较其差异,并采用卡方检验。结果卵泡膜细胞瘤中,3例(43%)T2WI呈等或低信号,4例(57%)呈稍高信号;肿瘤成分3例(43%)实性为主型,4例(57%)为囊实型;6例(86%)表现盆腔积液。卵泡膜纤维瘤中6例T2WI均呈等或低信号;肿瘤成分均为实性为主型;未见盆腔积液。纤维瘤中,3例(60%)T2WI呈等或低信号,2例(40%)呈稍高信号;肿瘤成分4例(80%)实性为主型;1例(20%)为囊实型;1例(20%)表现盆腔积液。T2WI信号、肿瘤成分和盆腔积液表现在3种类型之间的差异均有统计学意义(P分别=0.036、0.035、0.001)。T1WI信号、强化程度、退变等表现在3种类型之间差异均无统计学意义(P均>0.05)。结论不同病理类型的卵泡膜纤维瘤组肿瘤在T2WI信号、肿瘤成分和盆腔积液具有统计学差异,其MRI特征表现与病理类型具有一定的相关性。 Objective To analyze MRI features of different pathological types of fibrothecoma. Methods Histologi- cally proven 18 ovarian fibrothecomas were collected, including theeomas ( n = 7 ) , fibrotheeomas ( n = 6 ) , fibromas ( n = 5 ). all patients' MRI data were reviewed. MRI features, including signal, enhancement pattern, degeneration and so on, were retrospectively analyzed and compared among the three pathological types. Results Thecoma: 43% (3/7)showed hypointense to isointense on T2-weighted images, with 57% (4/7) showing hyperintense. 43% (3/7) presented as predom- inantly solid tumors, with 57% (4/7) appearing mixed solid and cystic, ascites was detected in 6 cases(86% ). Fibrothe- coma: 100% (6/6)showed T2W hyperintense and presented as predominantly solid tumors, no case(0% ) showed ascites. Fibroma: 60% (3/5)showed hypointense to isointense on T2-weighted images, with 40% (2/5) showing hyperintense. 80% (4/5) presented as predominantly solid tumors, with 20% (1/5) appearing as mixed solid and cystic, 20% (1/5) showed ascites. T2-weighted signal, Tumor component and ascites were statistically significant ( P respectively = 0. 036, 0. 035,0.001 ) among the three different pathological groups. T1 -weighted signal, the degree of enhancement and degenera- tion had no statistical significance ( P 〉 0.05 ) among the three different pathological groups. Conclusion Types of fihrothecoma. There is a certain correlation between the MRI features and its different pathological types, T2-weighted signal, tumor component and ascites were statistically significant among the three different pathological groups.
