体部不典型表皮样囊肿的MSCT与病理表现

MSCT and Pathologic Findings of Atypical Epidermoid Cyst of the Body

目的探讨体部不典型表皮样囊肿的MSCT特征及病理表现。方法回顾性分析经病理证实的33例患者共36个病灶的临床、MSCT及病理资料,其中26例行CT增强扫描。结果 31例单发,2例多发。病灶主要位于躯干部(17/36),其次为头面部(12/36),发生于四肢(7/36)者较少。囊壁增厚并强化者19个(52.8%),呈片状软组织密度并不均匀强化者7个(19.4%),病理显示继发感染、肉芽肿性炎或不同程度的异物巨细胞反应;呈高密度者6个(16.7%),其中伴钙化4个;完全钙化者4个(11.1%),病理示囊内同样为豆腐渣样物质,与角化物皂化及钙化或是含有较高浓度蛋白成分有关。结论体部不典型表皮样囊肿MSCT表现具有一定特征性,且能够反映病灶炎症、感染及钙化等病理特点。

Objective To explore MSCT imaging features and pathologic findings of the atypical epidermoid cyst of the body. Methods 36 lesions confirmed by pathology of 33 patients were analyzed retrospectively in clinical records, MSCT findings and pathological examinations. 25 patients underwent CT enhancement scanning. Results Single lesion was found in 31 cases, and multiple lesions were found in 2 cases. Lesions mainly occurred in the trunk （17/36） , followed by the head and face （ 12/36）, occurred in the limbs （7/36） were less. 19 cases（52.8% ） appeared as thick wall and rim enhancement. 7 cases（ 19.4% ） mimicked flaky soft tissue with heterogeneous enhancement. 6 cases （16.7%） showed nodules of high density, of which 4 cases with calcification. 4 cases （ 11.1% ） were completely calcified. Conclusion A- typical epidermoid cyst of the body has characteristic MSCT appearances, and can reflect the pathological features of the le- sion such as inflammation, infection and calcification. It can also help to deepen our knowledge of the rare signs of the body atypical epidermoid cyst and improve the ability of diagnosis of this disease.
