摘要
目的探讨白细胞介素17(IL-17)、人类软骨糖蛋白39(YKL-40)在慢性鼻-鼻窦炎发病中的可能作用机制及其与慢性鼻-鼻窦炎病情严重程度的相关性。方法选择本院2013年1月至2014年11月收治的50例慢性鼻-鼻窦炎患者纳入观察组,其中26例伴鼻息肉患者为A组,24例无鼻息肉患者为B组,另选取同期50例健康者为对照组。检测血清IL-17、YKL-40水平,并观察患者病情进展情况。结果观察组患者血清IL-17、YKL-40表达水平高于对照组(P<0.01),但A、B组患者血清IL-17、YKL-40表达水平比较差异无显著性(P>0.05),A组患者CT评分和鼻内镜评分均高于B组(P<0.01);IL-17表达水平与CT评分、鼻内镜评分呈正相关(r=4.521,4.281;P=0.019,0.021),YKL-40表达水平与CT评分、鼻内镜评分呈正相关(r=4.872,4.269;P=0.012,0.025),IL-17与YKL-40表达水平呈正相关(r=4.926,P=0.007)。结论 IL-17、YKL-40的表达与患者是否伴鼻息肉无关,而与慢性鼻-鼻窦炎的病情有关。IL-17与YKL-40可能在慢性鼻-鼻窦炎的发病及进展中协同发挥作用。
Objective To investigate the possible mechanism of interleukin 17(IL-17) and human chondrocyte protein 39(YKL-40) in chronic nasosinusitis patients and its correlation with the severity of chronic nasosinusitis. Method From January 2013 to November 2014, 50 patients with chronic nasosinusitis in our hospital were selected as observation group. among them, 26 cases with nasal polyps were group A, 24 cases without nasal polyps were group B, another 50 cases of healthy people were selected as control group. Serum levels of IL-17 and YKL-40 were detected, and the progress of progression disease were observed. Result The serum levels of IL-17 and YKL-40 in observation group were higher than control group(P〈0.01), but there were no significant differences in serum IL-17 and YKL-40 levels between group A and group B(P〉0.05). CT score and nasal endoscope score of group A were higher than group B(P〈0.01); IL-17 level was positively correlated with CT score and nasal endoscope score(r = 4.521, 4.281; P = 0.019, 0.021), YKL-40 level was positively correlated with CT score and nasal endoscope score(r = 4.872, 4.269; P = 0.012, 0.025), IL-17 level was positively correlated with YKL-40 level(r = 4.926, P = 0.007). Conclusion The expression of IL-17 and YKL-40 has nothing with nasal polyps, and is related to the chronic nasosinusitis. IL-17 and YKL-40 may play an important role in the pathogenesis and progression of chronic nasosinusitis.
