同步应用WI及脉象技术评估原发性高血压不同病理阶段心血管功能的研究 被引量:2

摘要 目的:探讨同步应用瞬时波强(wave intensity,WI)技术及脉象仪评估原发性高血压不同病理阶段的心血管功能。方法:对70例原发性高血压患者(内膜不增厚的A组36例及内膜增厚的B组34例)及33例健康人(对照组)行同步颈动脉WI检测及桡动脉脉象检测。采用彩色多普勒超声诊断仪行颈动脉WI检测,应用脉象仪检测桡动脉脉象参数,分析并比较WI参数及脉象参数。结果:高血压组瞬时加速度波强(W1)、负向波面积(NA)增高,WI曲线中W1波顶点至W2波顶点的时间(W1-W2)缩短,主波幅度(h1)、重搏前波幅度(h3)、降中峡幅度(h4)、h3/h1、h4/h1升高,重搏波幅度(h5)、h5/h1、脉图起始点到降中峡之间的时间(t4)降低。结论:WI和脉象同步检测为临床综合评估原发性高血压提供了新方法。 Objective:To explore the value of synchronously applying instantaneous wave intensity technique and pulsography in patients with primary hypertension of different pathological stages. Methods:70 cases of patients with primary hypertension were chosen in this research. The patients were divided into two groups:group A including 36 cases of the patients without carotid intima-media thickening and group B including 34 cases of the patients with carotid intima-media thickening. 33 cases of healthy people were chosen in the same period as the control group. Every case was examined by synchronously applying instantaneous wave intensity technique and pulsography. The indexes of WI of carotid artery were detected with Prosound α7 color Doppler ultrasonograph made in ALOKA company. And the parameters of pulse presentations of radial artery were detected with DDMX-100 pulsography. All the parameters were researched and compared. Results:In the indexes of W1,W1 and NA in hypertension group were significantly higher than that in control group;W1-W2 in hypertension group were significantly lower than that in control group.ln the parameters of pulse presentations,h1,h3,hn,h3/h1 and h4/h1 in hypertension group were significantly higher than that in control group;h5,h5/h1,t4 in hypertension group were significantly lower than that in control group (P〈0.05). Conclusion:Synchronously applying instantaneous wave intensity technique and pulsography provides a new way to evaluate primary hypertension.
