摘要
目的探讨6岁及以上哮喘儿童呼出气一氧化氮(Fe NO)水平及其与年龄、外周血嗜酸性粒细胞计数百分数(EOS%)和肺功能FEV1%、FEV1/FVC%的相关性。方法选择6岁及以上哮喘患儿共70例,按哮喘是否发作分为哮喘急性发作期及缓解期,以同期同年龄门诊健康查体的正常儿童21例为对照组。均行Fe NO、肺功能、血EOS检测。结果 1哮喘急性发作期组与缓解期组的Fe NO、外周血EOS差异无统计学意义(P>0.05),而两者与正常对照组差异均有统计学意义(P<0.05)。急性发作期组的FEV1%、FEV1/FVC%与缓解期组、正常对照组差异有统计学意义(P<0.05),缓解期组与正常对照组差异无统计学意义(P>0.05)。2相关性分析:Fe NO与6岁以上患儿年龄呈正相关(r=0.259,P<0.05)),与外周血嗜酸细胞百分比呈正相关(r=0.151,P<0.05);与FEV1/FVC%呈负相关(r=-0.151,P<0.05);Fe NO与FEV1%差异无统计学意义(r=-0.12,P>0.05)。结论Fe NO结合外周血EOS、气流阻塞评价指标即肺功能FEV1/FVC%,可以反映气道嗜酸性炎症及气道情况,帮助哮喘早期诊断,注意判读Fe NO结果应考虑年龄因素。
Objective To explore the level of fractional exhaled nitric oxide(FeNO) in asthmatic children aged 6 year old and above, and its correlation with age, peripheral blood eosinophilia count (EOS%), FEVI% and FEV1/FVC. Methods A total of 70 asthmatic children( age≥6 years) were enrolled and divided into acute exacerbation group and remission group with 21 healthy children as controls. Lung function, FeNO and EOS were measured for all children. Results (1)There was no statistical differences in FeNO and EOS between acute exacerbation group and remission group (P 〉 0.05 ), and there was significant statistical differences between asthmatic children and healthy children( P 〈 0.05 ). FEV1% and FEV1/FVC among acute exacerbation group, remission group and control group were different (P 〈 0.05 ) , and FEV1% and FEV1/FVC in the remission group were different from the control group(P 〉 0.05 ). (2)FeNO had positive relationship with age ( r = 0. 259, P 〈 0.05 ) and EOS% ( r = 0.151, P 〈 0.05 ), and had negative relationship with FEV1/FVC (r = -0. 151 ,P 〈0.05) ,and had no correlation with FEV1/FVC(r = -0.12,P 〉0.05). Conclusion FeNO can stand for the levels of eosinophilic inflammation. It is helpful for early diagnosis about asthma through FeNO and FEV1/FVC,but age could be taken into account.
