摘要
目的探讨系统化评估与风险预判为主导的临床护理专业能力培训提高临床护士评判性思维能力的效果。方法邀请美国危重症专科护士与资深护理教育专家作为培训教师。培训共3次,每次3个学时。3次培训内容分别为:理论讲授、情景模拟与小组讨论。对32名临床护士进行专业能力培训。在培训前后,采用评判性思维能力量表(Critical Thinking Disposition Imventory Chinese Version,CTDI-CV)对评判性思维能力进行评价;并采取反思日记评价护士培训后的感受与反馈。结果培训后,评判性思维能力总分,寻求真相、开放思想、系统化能力、思维自信心、求知欲等5个维度得分有提高,与培训前比较,差异有统计学意义(P<0.01)。反思日记提取出对现有临床思维模式的重新思索、对护士现有专业角色与价值的反思与对护理专业未来发展的思考3个主题。结论临床专业能力培训可以有效提升护士的评判性思维能力,增强了护士主动、评判、循证的思维意识,有利于护士专业角色与地位的提升。
Objective To evaluate the effects of professional competence training including systematic assessment and risk prejudging on improving critical thinking among nurses. Methods The training program was provided for 32 nurses. Critical Thinking Disposition Inventory-Chinese Version (CTDI-CV) and reflection diary were used to evalu- ate the effects and feedback. Results After training,the total score of CTDI-CV and 5 dimensions including truth seeking,open-minded,systematic ability,confidence of thinking and curiosity were significantly improved(P〈0.01 ). Three themes were extracted from the reflection diary:rethinking on current clinical thinking model,reflecting on cur- rent nurse professional role and value,and pondering on development of nursing science in the future. Conclusion Clinical professional competence training is an effective strategy to fortify critical thinking among nursing staff,which could enhance active,critical and evidence-based thinking and promote development of nursing profession.
