摘要
目的探讨简易空氧混合器在新生儿头罩氧疗中的临床疗效及安全性。方法将104例低氧血症患儿分为三组:A组(传统法组,32例)采用纯氧吸入,氧流量大于5 L/min;B组(低流量氧疗组,35例)采用纯氧低流量吸入,氧流量〈5 L/min;C组(空氧混合组,37例)采用自制的简易空氧混合器,根据目标氧浓度调节氧气及空气的流量,氧流量〈5 L/min,空氧混合总流量大于5 L/min。于头罩氧疗3小时后,对比三组的治疗有效率及血气分析结果。结果三组的治疗有效率比较无统计学差异(P〉0.05),但A组高氧分压发生率高于C组(χ^2=38.638,P〈0.05),B组二氧化碳潴留发生率高于C组(χ^2=12.413,P〈0.05)。结论使用简易空氧混合器行头罩吸氧法的疗效确切,副作用少,临床操作性强,值得基层医院推广。
Objective To explore the safety and effects of simple air-oxygen blender in oxygen therapy via head box of newborns.Methods 104 cases of newborns with hypoxemia were divided into three groups. Group A(32 cases) inhaled pure oxygen with flow rate 5L/min, group B(35 cases) inhaled pure oxygen with flow rate 5 L/min, and group C(37 cases) received inhalation by simple air-oxygen blender made by ourselves to regulate the oxygen flow and air flow to achieve the target oxygen concentration, with oxygen flow rate 5L/min and total flow rate 5 L/min. Three hours after treatment, the curative effective rate and ABG results were compared among 3 groups.Results There was no statistical difference among three group in the curative effective rate(P〈0.05). The incidence rate of high oxygen partial pressure of group A was significantly higher than that of group C(χ^2= 38.638, P〈0.05), and the incidence rate of carbon dioxide retention of group B was significantly higher than that of group C(χ^2= 12.413, P〈0.05). Conclusions Simple air-oxygen blender in oxygen therapy via head box has exact effect, and few adverse reactions; also, it's easy to operate in clinic, and worthy of promotion in primary hospital.
