PDCA循环管理在防范患者跌倒护理中的应用

Application of PDCA cycle management in the nursing care of preventing patients from falling down

目的探讨PDCA循环管理在提高病房护士防范患者跌倒护理中的应用效果。方法通过对2012年武警某部医院某科发生的4例跌倒患者(均为重度高危患者)的临床资料进行收集、分析、归纳和总结,科室于2013年导入PDCA循环管理模式即以计划、实施、检查、处理的科学程序,培训护士防跌倒相关知识,并进行以提高其临床执行能力为目的的循环管理。结果 (1)跌倒发生率从5.97%降至0.86%,但差异无统计学意义(P=0.061);(2)与PDCA循环管理实施前相比,患者及家属对护士健康宣教、主动服务意识、跌倒护理的满意度分别从56.72%、62.69%、59.70%上升至82.76%、94.83%、87.93%,差异均有统计学意义(P<0.0001)。结论 PDCA循环管理的应用提高了病房护士防范患者发生跌倒的能力,丰富了护士防跌倒护理的知识,提高了防范意识,增强了护士执行能力,有效降低了跌倒发生率,增进了护患感情。

Objective To investigate the application effect of PDCA cycle management in improving the ability of ward nurses to prevent patients from falling down. Methods Through collection, analysis, induction and summary of the clinical data of 4 patients with high risk of falling down at a department of armed police forces＇ hospital in 2012, PDCA mode, that is plan, implementation, inspection, processing of science program was employed in the department in 2013 with an aim to train nurses of fall-prevention knowledge, and improve their execution ability via the PDCA cycle management. Results（1） The incidence rate of falling down decreased from 5.97% to 0.86%, however, the difference failed to get a statistical significance（P=0.061）;（2） Compared with the satisfaction degree of patients and their relatives before the PDCA cycle management application, the satisfaction degree for health education, consciousness of initiative service, tumbling prevention nursing raised from 56.72%, 62.69%, 59.70% to 82.76%, 94.83%, 87.93%, respectively, and the differences were all statistically significant（P〈0.0001）. Conclusions The application of PDCA cycle management enhanced the ability of ward nurses to prevent patients from falling down, enriched fall prevention knowledge of nurses, improved their awareness, boosted the nurses＇ ability to execute, reduced the incidence rate of falling down, and harmonized the relationship between patients and nurses.
