不同负压值对创面负压封闭引流的效果观察

The effects of different negative pressure on the treatment of vacuum sealing drainage

目的：探讨不同负压值对创面负压引流（VSD）的效果观察，并为VSD治疗提供合适的负压值。方法：选择2014年1月～2015年1月在我科住院并使用VsD治疗软组织损伤的患者225例，其中75例为1块敷料单根置管者，75例为2～4块敷料多根置管、伤口达浆膜层者，另75例为≥5块敷料多根置管、特殊感染者，每7S例又分为3组，每组负压分别调至-125，-188，-450mmHg。观察各组负压引流治疗期间更换引流物次数、创面清洁时间及愈合时间。结果：1块敷料单根置管者，在负压-125mmHg时更换引流物次数、创面清洁时间、愈合时间都较其他两种负压少;2～4块敷料多根置管、伤口达浆膜层者采用负压值为-188mrnHg时，效果最佳；而≥5块敷料多根置管、特殊感染者则采用一450mmH售时效果最好。各组比较均有显著统计学意义（P〈0．05）。结论：不同负压值对VSD的效果有显著影响，不同类型创面及置管情况采用不同负压可以有效减少重新置管的次数，缩短创面清洁时间、创面愈合的时间，提高治疗效果，减轻患者的经济负担。

Objective I explore the effects of different negative pressure has on the treatment of VSD and choose appropriate negative pressure. Methods ：Take 225 patients hospitalized in our department between Jan. 2014 and jan. 2015 and who have experienced VSD treatment for soft tissue injury as our subject, including 75 patients with one dressing and one tube, 75 patients with 2 to 4 dressings and several tubes whose serosa have been wounded, 75 special infec- ted patients with more than 5 dressings and several tubes. Every 75 patients were divided into three groups and three different negative pressure were applied on different groups, including -125, -188 and -450 mmHg, Then watch how often the patients need to change the implant drainage, how long it takes to clean the wound and how long the wound would heal. Results ： Patients with one dressing and one tube have fewer implam drainage, less wound cleaning lime and healing time when the negative pressure was - 125 mmHg; patients with 2 to 4 dressings and several tubes whose serosa have been wounded have the best treatment effect when the negative pressure was - 188 mmHg; those special infected patients with more than 5 dressings and several tubes have the best treatment effect when the negative pressure was -450 mmHg. The comparison has statistic meaning （P 〈 0.05 ）. Conclusion：Different negative pres- sure has significant influence on VSD treatment effect. If different negative pressure is applied on the patients with different wounds and tube implant, the patients will have fewer tube implants, less wound cleaning time and less healing time. The VSD treatment will have better effects and the economic burden of patients will be lightened.
