搭建在医院社区共建信息化平台上的脂肪肝临床路径探索

Clinical pathway exploration of fatty liver in information construction platform based on hospital and community

目的探索脂肪肝临床路径的新模式。方法通过对符合脂肪肝诊断标准的80例脂肪肝患者在实施临床路径管理前后的脂肪肝知晓率、依从性等情况的对比,评估医院社区信息一体化脂肪肝临床路径管理的实用性和有效性。结果临床路径实施后,本院管理的脂肪肝患者的总人数新增了80人,并且与临床路径实施前相比较,80例脂肪肝患者的依从性、脂肪肝知晓率都有明显改善,各临床指标包括体重、体重指数、腰围、空腹血糖、甘油三酯、胆固醇、谷丙转氨酶、谷草转氨酶改善明显,差异均有统计学意义(t分别=3.66、4.32、4.65、4.00、3.12、7.21、7.50、3.08,P均<0.05)。结论搭建在医院-社区共建信息化平台上的脂肪肝临床路径能增强脂肪肝患者对脂肪肝的认识,提高脂肪肝的管理水平,具有良好的临床效应。

Objective To explore a new model of clinical pathway of fatty liver. Methods Clinical pathway of fatty liver was applied to 80 patients with fatty liver disease, the practicability and effectiveness of the management of the clinical pathway of fatty liver were evaluated. Results After the implementation of the clinical pathway, the total number of fatty liver patients that were managed in our hospital added 80 cases. Compared to before, the compliance, fatty liver awareness of 80 cases with fatty liver were improved significantly, the clinical indicators including weight, body mass index, waist circumference, fasting blood glucose, triglyceride, cholesterol, valley pyruvic transaminase and aspartate transaminase were also improved significantly（t=3.66, 4.32, 4.65, 4.00, 3.12, 7.21, 7.50, 3.08,P0.05）. Conclusion The clinical pathway of fatty liver in information construction platform based on hospital and community can enhance the knowledge of fatty liver, improve the management level of fatty liver and has a good clinical effect.
