喹硫平联合碳酸锂治疗双相障碍躁狂的临床效果及安全性评价

Evaluation of clinical effect and safety of quetiapine combined with lithium for the treatment of bipolar mania

目的探讨喹硫平联合碳酸锂治疗双相障碍躁狂发作的临床效果及安全性。方法 164例双相障碍躁狂患者,分为观察组和对照组各82例,观察组采用喹硫平联合碳酸锂治疗,对照组采用喹硫平联合安慰剂治疗,采用双盲方法应用BRMS量表于治疗前和治疗1、2、4及6周后评定临床效果;采用副反应量表评价治疗后的不良反应发生情况。结果两组患者治疗后各时点BRMS评分均明显降低,差异有统计学意义(P<0.05);且观察组明显优于对照组,差异有统计学意义(P<0.05)。治疗后各时点观察组有效率均高于对照组(P<0.05)。治疗过程中两组均发生不良反应,但差异无统计学意义(P>0.05)。结论双相障碍躁狂发作患者采用喹硫平联合碳酸锂药物进行治疗能够快速有效地缓解患者临床症状,效果良好,且不增加不良反应发生率。

Objective To evaluate the clinical efficacy and safety of quetiapine combined with lithium carbonate in the treatment of bipolar disorder. Methods One hundred and sixty four patients with bipolar disorder manic were randomly divided into observation group or control group,82 in each group. The observation group received quetiapine combined with lithium carbonate while the control group was treated with quetiapine combined with placebo. A double blind method with BRMS scale was used to compare before treatment with after 1,2,4 and 6 weeks of treatment to evaluate the curative effect and the changes in clinical symptoms. The adverse reactions were also evaluated by using a treatment emergent symptom scale （TESS）. Results The BRMS scales at different time point in the two groups were significantly decreased after treatment （ P 〈 0. 05 ）, and the decreased degree in the observation group was more than that in the cuntrol group （P 〈 0.05）. The curative effects at different time point after treatment in the observation group were all better than that in the control group （P 〈 0. 05）. There were adverse reactions in the both groups but the difference was no statistically significant （P 〉 0. 05 ）. Conclusion Quetiapine combined with lithium carbonate in the treatment of patients with bipolar disorder manic episode can effectively relieve the clinical symptoms, has a good effect and does not increase the incidence of adverse reactions.
