青年女性乳腺癌分子分型及其与临床病理特征的关系 被引量:7

摘要 目的分析35岁以下的青年女性乳腺癌患者分子分型及其与临床病理特征和预后情况的关系。方法回顾性分析2008年1月至2013年12月在仙居县妇幼保健院乳腺外科手术治疗且病理确诊为乳腺癌的104例35岁以下女性患者的临床资料,采用免疫组化方法进行分子分型,并分析其与临床病理特征和预后情况之间的关系。结果此次研究中青年乳腺癌患者占同期乳腺癌的7.60%(104/1 368),包括1uminal A型54例(51.92%)、luminal B型16例(15.38%)、人表皮生长因子-2型(HER-2+型)13例(12.50%)、三阴型乳腺癌21例(20.19%)。各亚型乳腺癌患者在触及肿块至就诊时间、病理分型上的差异均有统计学意义(均P<0.05)。Luminal A、luminal B、HER-2+型和三阴型4组的无病生存率分别为96.23%、87.50%、92.31%和85.71%,4组分子亚型之间的5年无病生存率和总生存率差异均无统计学意义(均P>0.05)。结论 35岁以下青年乳腺癌患者的分子分型与触及肿块至就诊时间和肿瘤病理分型密切相关。 Objective To explore the molecular subtypes of breast cancer in young women and their relationship with clinicopathological features and prognosis. Methods Retrospective analysis was conducted on the clinical data of 104 young women with breast cancer ( ≤35 years old) treated in department of breast surgery of Xianju County Maternal and Child Health Care Hospital from January 2008 to December 2013. Molecular subtypes were graded using immunohistochemical method, and their relationship with clinicopathological features and prognosis of breast cancer was analyzed. Results Young breast cancer patients in this study accounted for 7.6% (104/1368) in the same period, including 54 cases (51.92%) of luminal A subtype, 16 cases (15.38%) of luminal B subtype, 13 cases (12.50%) of HER-2 + subtype, and 21 cases (20.19%) of triple-negative subtype. There were statistically significant differences among patients with different molecular subtypes regarding the interval between palpable mass and treatment and tumor pathological subtype ( both P 〈 O. 05 ). The disease-free survival (DFS) rates of patients with Luminal A, luminal B, HER-2 + and triple negative breast cancer were 96.23% , 87.50% , 92.31% and 85.71% , respectively, and there were no significant differences among patients in 5-year DFS rate and overall survival (OS) rate ( both P 〉 0.05 ). Conclusion The interval between palpable mass and treatment and tumor pathological subtype are closely related with molecular subtypes of young women with breast cancer.
