摘要 目的对存在营养不良的肝功能衰竭患者制定相应营养干预措施,以了解营养干预对肝功能衰竭患者的疗效。方法选取2014年1-8月在解放军第81医院全军肝病中心住院的44例营养不良肝功能衰竭患者为研究对象,按入院先后将其分为观察组和对照组两组,对照组给予内科综合治疗,观察组在常规内科治疗基础上针对营养评估的结果进行营养干预,观测并比较干预前与干预后两组患者的营养生化指标、人体测量营养相关指标、主观全面评价法(subjective global assessment,SGA)评分及并发症发生情况。结果干预前,两组患者的营养生化指标、人体测量指标及SGA评分的差异均无统计学意义(均P>0.05);干预后,观察组患者的营养生化指标[除白蛋白(albumin,ALB)外]、身体质量指数(body mass index,BMI)及SGA评分均优于对照组(均P<0.05);干预后,观察组患者的营养生化指标[除胆固醇(cholesterol,TC)外]、BMI、上臂围(mid-arme circumference,MAC)及SGA评分均优于干预前(均P<0.05);对照组患者的SGA评分优于干预前(P<0.05),其余指标与干预前比较,差异均无统计学意义(均P>0.05)。干预后,观察组患者并发症的发生率低于对照组,差异有统计学意义(P<0.05)。结论营养干预有利于改善营养不良肝功能衰竭患者的营养状态,减少并发症的发生。 Objective To make targeted nutritional intervention measures for the dystrophic patients,in order to understand the effect of nutritional intervention in liver failure patients.Methods By convenience sampling, 44 liver failure patients were selected and divided into observation group and control group. The control group received comprehensive treatment, while the observation group applied with nutritional intervention. The nutritional and bio- chemical indicators, anthropometric nutrition related indicators, subjective global assessment(SGA) and incidence of complications were compared.Results Before intervention, there was no statistical significance of nutritional and bio- chemical indicators,anthropometric nutrition related indicators and SGA socre showed between two groups(all P〉 0.05).After intervention,the nutritional and biochemical indicators(except albumin),BMI and SGA scores in observation group were better than that of control group (all P〈0.05).After intervention,the nutritional and biochemical indicators(except cholesterol), BMI, MAS and SGA score in observation group were better than those before intervention (all P〈0.05).The SGA score in control group was better than that before intervention (P;0.05).There was no statistical significance showed in other indexes compared with before intervention (all P 〉 0. 05 ) . After intervention, the complication rate in observation group was lower than that of control group (P〈0.05).Conclusion Nutritional inter- vention can improve the nutritional status of the patients with liver failure, and reduce the complications.
