摘要
目的分析结核菌特异性抗原ESAT-6及CFP-10在肺结核及肺炎患者中的诊断价值。方法将48例肺结核患者作为观察组,30例肺炎患者作为对照组,分别进行PPD及ELISA实验,对两组患者的结核菌特异性抗原在两种疾病中的诊断价值进行分析。结果经ELESA实验检测,观察组结核菌阳性率、特异度及灵敏度分别为87.50%、93.75%、89.58%;对照组结核菌阳性率、特异度及灵敏度分别为60.00%、66.67%、70.00%;组间比较差异具有统计学意义(P<0.05)。经PPD实验检测,观察组结核菌阳性率、特异度及灵敏度分别为45.83%、50.00%、47.92%;对照组结核菌阳性率、特异度及灵敏度分别为46.67%、50.00%、53.33%;组间比较差异无统计学意义(P>0.05)。结论采用ELISA法对肺结核及肺炎患者ESAT-6及CFP-10抗原进行检测可鉴别两者的不同,与PPD法相比,ELISA法在肺结核及肺炎的鉴别中诊断价值更优。
Objective To evaluate the value of Mycobacterium tuberculosis specific antigen ESAT-6 and CFP-10 in the diagnosis of pulmonary tuberculosis and pneumonia patients. Methods 48 cases of patients with pulmonary tuberculosis as the observation group,30 cases of patients with pneumonia as control group,PPD and ELISA experiments were performed and the diagnostic value of two groups of patients with tuberculosis specific antigen in the two kinds of diseases were analyzed. Results By ELISA assay to observe the Mycobacterium tuberculosis positive rate,specificity and sensitivity were 87.50% and 93.75% and 89.58%. Control group TB positive rate,specificity and sensitivity were 60.00%, 66.67%, 70.00%,inter group had significant difference(P〈0.05). The PPD test,observation group TB positive rate,sensitivity and specificity of 45.83%,50.00%, 47.92% respectively,control group TB positive rate,specificity and sensitivity were 46.67%,50.00%,53.33%,and inter group had no significant difference(P〉0.05). Conclusion ELISA method can be used to detect the ESAT-6 and CFP-10 antigen in patients with pulmonary tuberculosis and pneumonia,and it can be used to distinguish the difference between ELISA and PPD.
