法与标准实施对精神科住院患者约束保护的效果分析

Effect of Mental Health Law on the Protection of the Inpatients in Department of Psychiatry

目的通过临床资料的回顾性调查,总结分析在我国精神卫生领域具有里程碑意义的精神卫生法及《三级精神病医院评审标准》的实施,对精神科住院患者约束保护护理管理质量的影响。方法通过法与标准实施前(对照组)后(观察组)的约束保护护理措施的落实,分析比较两组措施对精神科住院患者约束保护的护理管理质量影响。结果两组约束保护的医嘱正确执行合格率、护理程序应用合格率、护理记录合格率、护患纠纷发生率其差异有统计学意义;两组约束率、依从率、平均每人次约束时间、平均每人约束次数差异有统计学意义;两组护理人员历次考核中对约束保护相关理论及实践操作的合格率比较差异有统计学意义。结论精神卫生法及三级精神病专科医院评审标准的实施有利于提高精神科住院患者约束保护的临床护理管理质量。

Objective By retrospective investigation of clinical data, the influence of mental health law which is of milestone significance in the field of mental health and ＂three levels of psychiatric hospital accreditation standards＂ on nursing management quality of protection and constraint of inpatients in department of psychiatry are summarized and analyzed. Methods Nursing management quality of inpatients in department of psychiatry was an- alyzed through contrasting implementation of restraint nursing measures before the implementation of the law and the standard （ control group） and after the implementation of the law and the standard （ observation group ）. Results The correct rate of medical order implementation, nursing procedures, nursing records, and the incidence of nurs- ing dispute between the two groups were of statistical significance; The incidence of constraints, compliance rate, the average time of constraints per person, the average number of constraints per person were of statistical signifi- cance; The pass rate of the past relevant constraint theoretical and practical exams between the two groups was of statistical significance. Conclusion The implementation of Mental health law and three levels of psychiatric hospi- tal accreditation standards improved the clinical nursing management quality of constraint and protection of inpatientsin department of psychiatry.
