摘要
目的探讨剪切波弹性成像(SWE)彩色图像特征评价乳腺纤维腺瘤的价值。方法回顾性分析经病理证实为乳腺纤维腺瘤患者98例(115个病灶)的SWE彩色图像特征,根据颜色六分法、弹性彩色图像分类法和弹性均质性评分评估病灶SWE彩色图像特征,测量病灶弹性模量值并分析其与三种评估方法的相关性。结果根据乳腺影像报告与数据系统(BI-RADS)诊断标准,共68个病灶需要活检,颜色六分法、弹性彩色图像分类法和弹性均质性结合BI-RADS活检率分别为20.9%、21.7%和36.5%,并且64.7%、63.2%和38.2%病灶避免了不必要的穿刺活检。三种彩色图像特征评估结果与最大弹性模量值、弹性标准差值均有显著相关性(P<0.01)。结论 SWE彩色图像特征评估作为常规超声的重要补充,可减少对于乳腺纤维腺瘤的活检率。
Objective To explore the value of color image features of shear-wave elastography(SWE)in assessing breast fibroadenoma.Methods Color image features of 115 lesions in 98 patients with breast fibroadenoma undergoing SWE were retrospectively analyzed,which were assessed with the methods of six-point color score,four-colour overlay pattern classification and elastic homogeneity.Elastic mould values of lesions were measured and their correlations with the three methods mentioned above were analyzed.Results According to the standards of breast imaging reporting and data system(BI-RADS),the biopsy was needed in 68 lesions.Combined with BI-RADS,the rates of biopsy indicated by the results of the three methods were 20.9%,21.7% and 36.5%,respectively.There were 64.7%,63.2% and 38.2% of the lesions avoided unnecessary biopsies.The Emaxand Std were closely correlated to the results of three assessment methods(P〈0.01).Conclusion Color image feature evaluation of SWE may be taken as an important complementary for reducing biopsy rate of breast fibroadenoma.
