摘要
目的探讨射频消融联合臭氧治疗神经根型颈椎病的临床效果。方法采用射频消融联合臭氧治疗颈椎病36例。射频消融:在C型臂X射线机辅助下定位穿刺,置入射频电极,测阻抗在150~250Ω;分别用高频50Hz、0.5~2.0mA低电流行感觉测试,低频2Hz、0.5~2.5mA高电流行运动测试;若无异常反应后,则行60℃、70℃和80℃各60s,85℃治疗120s。臭氧治疗:射频结束后,拔出电极,抽臭氧(60mg/ml)共3~5ml,缓慢推注。术后24h、1周、1个月和6个月随访疗效。结果术后24h、1周、1个月和6个月的有效率分别为97.2%、94.4%、94.4%和94.4%,优良率分别为94.4%、88.8%、80.5%和86.1%。随访期未见并发症。结论射频消融联合臭氧治疗神经根型颈椎病安全、有效。
Objective To investigate the efficacy of radiofrequency ablation combined with ozone in treating cervical spondylotic radiculopathy.Methods Thirty-six patients with cervical spondylotic radiculopathy were treated with radiofrequency ablation combined with ozone.The radiofrequency ablation was performed with an electrode of impedance 150-250Ω,which was localized and inserted under an assistance of C-arm X-ray.When there were no abnormal responses to sensory testing with high frequency low current(50Hz,0.5-2.0mA)and movement testing with low frequency high electric current(2Hz,0.5-2.5mA),the radiofrequency ablations were carried out with60℃,70℃ and 80℃for 60 s,which were followed by 85℃ for 120 s.After completion of ablations,ozone 3-5ml(60mg/ml)was injected slowly.Clinical efficacy was evaluated at 24 hours,1week,1month and 6months after treatment.Results At 24 hours,1week,1month and 6months after treatment,the effectiveness rates were 97.2%,94.4%,94.4% and 94.4%,and excellence rates were94.4%,88.8%,80.5% and 86.1%,respectively.No complications were seen during follow up.Conclusion The radiofrequency ablation is safe and effective in the treatment of cervical spondylotic radiculopathy.
