摘要 目的:研究Bmi-1与P27在食管癌组织中的表达并分析其与食管癌临床病理特征的关系,探讨其在食管鳞状细胞癌发生、发展过程中的生物学意义。方法:用免疫组化SABC法检测24例食管正常鳞状上皮(A组)、33例食管鳞状上皮内瘤变[其中低级别19例(B组),高级别14例(C组)]和63例食管鳞状细胞癌组织(D组)中Bmi-1与P27的表达,分析其与食管鳞状细胞癌临床病理特征的关系以及两者之间的相关性。结果:Bmi-1在A、B、C、D四组中阳性表达率依次增高。其中A组与B组、A组与C组、B组与C组之间,Bmi-1差异均无统计学意义(P>0.05);A、B、C三组与D组之间,Bmi-1差异均有统计学意义(P<0.05)。Bmi-1的表达与淋巴结转移、TNM分期密切相关(P<0.05),而与性别、年龄、肿瘤部位、浸润深度、病理组织分级及病理形态无关(P>0.05)。P27在A组中高表达,在B组、C组、D组中表达率逐渐减低。A组与B组、A组与C组、B组与C组之间,P27差异均无统计学意义(P>0.05);A、B、C三组与D组之间,P27差异均有统计学意义(P<0.05)。P27的表达与性别、年龄、肿瘤部位、浸润深度、病理组织分级、病理形态、淋巴结转移及TNM分期之间差异无统计学意义(P>0.05)。D组中P27表达随Bmi-1的表达增强而逐渐减弱,呈负相关(r_s=-0.256,P<0.05)。结论:Bmi-1与P27的差异表达可能与食管鳞状细胞癌的发生发展及转移有关,其有望作为评价食管鳞状细胞癌生物学行为和预后的新指标。 AIM: To investigate the expressions of Bmi-1 and P27 in the esophageal squamous cell carcinoma( ESCC) and its relationship with the clinicopathological features and expression correlation between Bmi-1 and P27 and to explore the roles in the occurrence and development process for ESCC.METHODS: The expressions of Bmi-1 and P27 were detected by the SABC immunohistochemistry in24 cases of normal esophageal mucosal epithelia( group A),33 cases of intraepithelial neoplasia[including 19 cases of low grade( group B),high grade14 cases( group C) ] and 63 cases of ESCC( group D),and the correlation between the expressions of Bmi-1 and P27 and their relations with the clinicopathologic features of ESCC were analysed. RESULTS: The positive expression rate of Bmi-1 was increased gradually in group A,B,C and D. Among the three groups compared with any two,group A and B,group A and C,group B and C,there were no statistically significant differences( P〉0. 05); compared with the group D,the expressions of Bmi-1 in group A,group B,group C were not statistically significant difference( P〈0. 05). Bmi-1 expression was closely related to lymph node metastasis,TNM stage,and the difference was statistically significant( P〈0. 05),while gender,age,tumor location,depth of invasion,histological grade and pathological type have no statistically significant differences( P〉0. 05). The expression of P27 in group A was high,but the expression rates were reduced gradually ingroup B,C and D. Among the there groups compared with any two,group A and B,group A and C,group B and C,there were no statistically significant differences( P〉0. 05); compared with the group D,the expressions of P27 in group A,group B,group C were statistically significant difference( P〈0. 05). With gender,age,tumor location,depth of invasion,histological grade,pathological type,lymph node metastasis and TNM stage,the difference was not statistically significant( P〉0. 05). The expressions of Bmi-1 and P27 in group D were negatively correlated( rs=- 0. 256,P〈0. 05). CONCLUSION: Increased expression of Bmi-1 and decreased expression of P27 may play an important role in ESCC and be expected as the judge ESCC biological behavior and prognosis of new indicates.
